摘要
目的为进一步提高家庭医生团队服务能力,充分发挥其“健康守门人”的作用,为制定更完善的家庭医生团队制度提供科学依据。方法上海市金山区朱泾社区卫生服务中心自2020年初开始推行家庭医生团队医防融合一体化建设,通过收集一体化建设前后家庭医生团队服务资料,分析家庭医生一体化模式建设优势及其对医疗改革的促进作用。结果家庭医生团队医防融合一体化建设促进了家庭医生团队中学科的融合,弥补了不同科学领域间的学科裂痕,优化了管理网络,有效地整合了服务资源,切实提升了工作效率,规范了慢性病管理,保证了新冠肺炎等重大公共卫生事件的处置效率,提升了居民获得感。结论社区家庭医生团队医防融合一体化建设成效显著,值得推广。
Objective To further improve the service ability of family doctor team,give full play to its role of“health gatekeeper”,and provide scientific basis for formulating a more perfect family doctor team system.Methods Zhujing Community Health Service Center in Jinshan District of Shanghai City has implemented the integrated construction of family doctor team medical and preventive integration since the beginning of 2020.By collecting the service data of family doctor team before and after the integrated construction,the advantages of family doctor integration model and its role in promoting medical reform were analyzed.Results Through the implementation of the construction of medical and preventive integration of family doctor team,family doctors team has integrated the disciplines,made up the disciplinary gaps among different scientific fields,optimized the management network,effectively integrated the service resources,effectively improved the work efficiency,standardized the management of chronic diseases,and ensured the efficiency of dealing with major public health emergencies,such as COVID-19 epidemic.It has improved residents’sense of access.Conclusion The medical and preventive integration model of community family doctor team has achieved remarkable results and is worth popularizing.
