摘要
目的评价单用“醒脑开窍”针刺法或“醒脑开窍”针刺法联合其他疗法治疗中风后吞咽障碍的疗效。方法检索中国生物医学文献数据库、中国知网、万方数据资源系统、中文科技期刊数据库、PubMed自建库截至2020年6月25日的文献。用Cochrane协作网推荐的“偏倚风险评估工具”评价文献质量,用Rev Man5.4软件进行Meta分析。结果纳入文献19篇,总样本量1874例。(1)总有效率比较:“醒脑开窍”针刺法高于常规针刺法(P<0.001);“醒脑开窍”针刺法与吞咽治疗仪对比无统计学差异(P=0.91);“醒脑开窍”针刺法联合康复训练高于单纯康复训练(P<0.001);“醒脑开窍”针刺法联合吞咽治疗仪高于单用吞咽治疗仪(P<0.001);“醒脑开窍”针刺法联合西药高于单用西药(P<0.001)。(2)标准吞咽功能评定量表评分比较:“醒脑开窍”针刺法联合康复治疗优于康复治疗(P=0.001)。(3)吞咽障碍特异性生活质量量表评分比较:“醒脑开窍”针刺法联合康复治疗优于康复治疗(P=0.001)。结论单用“醒脑开窍”针刺法或与其他疗法联合能提高中风后吞咽障碍患者的临床疗效和生活质量,但需更多高质量、大样本、多中心的随机对照试验证实。
Objective To systematically evaluate the efficacy of Xingnao Kaiqiao acupuncture only oritcombined with other therapies in the treatment of post-stoke dysphagia.Methods The relative literature was retrieved from the databases of China Biology Medicine,China National Knowledge Infrastructure,WanFang and VIP.The retrieval time span was from the database establishment to June 25th 2020.The"Risk of Bias Assessment Tool"recommended by the Cochrane Collaboration was used to evaluate literature quality,and Rev Man 5.4 software was used for Meta analysis.Results 19 references were included,with a total sample size of 1874.(1)Comparison of total effective rates:the total effective rate of Xingnao Kaiqiao acupuncture was higher than that of conventional acupuncture(P<0.001).There was no difference between the total effective rate of Xingnao Kaiqiao acupuncture and that of the swallowing therapeutic apparatus(P=0.91).The total effective rate of Xingnao Kaiqiao acupuncture combined with rehabilitation training was higher than that of rehabilitation training(P<0.001).The total effective rate of Xingnao Kaiqiao acupuncture combined with swallowing therapeutic apparatus was higher than that of the single swallowing therapeutic apparatus(P<0.001).The combination of Xingnao Kaiqiao acupuncture with western medicine was more effective than western medicine alone(P<0.001).(2)Comparison of scores of Standardized Swallowing Assessment:The combination of Xingnao Kaiqiao acupuncture with rehabilitation training was superior to the rehabilitation treatment singly(P=0.001).(3)Comparison of swawowing-quality of life scores:The combination of Xingnao Kaiqiao acupuncture with rehabilitation training was superior to the rehabilitation treatment singly(P=0.001).Conclusion Xingnao Kaiqiao acupuncture alone or in combination with other therapies can improve the clinical efficacy and quality of life in patients with dysphagia after stroke,but more high-quality,large-sample,multi-center randomized controlled trials are needed to confirm this conc
