慢性移植物抗宿主病患者循环内皮祖细胞数量分析

Quantitative analysis of circulating endothelial progenitor cells in patients with chronic graft-versus-host disease

摘要 目的:分析异基因造血干细胞移植(allo-HSCT)后慢性移植物抗宿主病(cGVHD)患者外周血内皮祖细胞(EPC)的数量变化,探讨cGVHD与内皮系统损伤修复之间的联系。方法:收集23例allo-HSCT后cGVHD患者的外周血,以同期无cGVHD患者(15例)及健康供者(20例)作为对照,通过流式细胞术分析EPC数量;并对不同分类分级cGVHD患者EPC数量进一步分析。结果:与对照组比较,cGVHD组及无cGVHD组外周血EPC含量均显著减少[(439±238.7)个/mL vs (1130±254.0)个/mL,P<0.001;(869±407.9)个/mL vs (1130±254.0)个/mL,P=0.014],且cGVHD组EPC数量显著少于无cGVHD组(P<0.001)。23例cGVHD患者中,广泛型cGVHD组(5例)与局限型cGVHD组(18例)EPC数量比较,差异无统计意义[(334±288.2)个/mL vs (476±216.3)个/mL,P=0.218];中度cGVHD组(16例)较轻度cGVHD组(7例)EPC数量明显减少[(368±207.2)个/mL vs (601±240.0)个/mL,P=0.028]。结论:cGVHD患者外周血EPC数量显著减少,且随严重度增加,其数量进一步下降,提示内皮系统的损伤及修复能力下降可能参与了allo-HSCT后cGVHD发病。 Objective: To explore the level of circulating endothelial progenitor cells(EPC) in patients who underwent chronic graft-versus-host disease(cGVHD) after allogeneic hematopoietic stem cell transplantation(allo-HSCT). Methods: A total of 23 patients with cGVHD, 15 patients without cGVHD and 20 healthy donors were enrolled in the study. Blood samples were collected and analyzed by flow cytometry. EPC was defined by triple expression of CD34/CD133/KDR. Results: Compared to healthy donors, the circulating EPC levels were significantly decreased in patients after allo-HSCT with cGVHD or without cGVHD([439±238.7]cells/mL vs [1130±254.0]cells/mL, P<0.001;[869±407.9]cells/mL vs [1130±254.0]cells/mL, P=0.014). The EPC level was even much lower in patients with cGVHD than that in patients without cGVHD(P<0.001). In 23 patients with cGVHD, the EPC level was not significantly reduced in 5 patients with extensive cGVHD than that in 18 patients with limited cGVHD([334±288.2]cells/mL vs [476±216.3]cells/mL, P=0.218), but it was significantly lower in 16 patients with moderate cGVHD than that in 7 patients with mild cGVHD([368±207.2]cells/mL vs [601±240.0]cells/mL, P=0.028). Conclusion: Our results demonstrate that the number of circulating EPC is significantly decreased in patients with cGVHD, and it is decreased with the severity scoring. Dysfunction of endothelium and decreased repair ability may participate the pathogenesis of cGVHD.
