摘要
目的本文旨在分析老年糖尿病足患者足背动脉血流量、脂蛋白a水平变化及临床意义,为治疗该病症提供临床依据。方法选取西部战区总医院2017年8月-2020年8月住院治疗的老年糖尿病足患者204例设为观察组,另外选取同时期西部战区总医院长期门诊且未合并糖尿病足2型糖尿病患者200例设为对照组。收集2组患者临床、影像学及实验室资料,比较不同组别、Wagner分级及治疗前后患者足背动脉血流量和脂蛋白a水平,采用多元Logistic回归模型分析影响糖尿病足出现的相关因素。结果对照组和观察组患者的糖尿病生化指标(TG、HDL-C、LDL-C、TC、FPG)及脂蛋白相关指标(Apo A、ApoB、ApoE)比较差异无统计学意义(P> 0.05);观察组左/右侧足背动脉血流量和左/右侧足背动脉内径水平低于对照组,而脂蛋白a水平高于对照组(P<0.05);在观察组中,随着Wagner分级的增高,患者左/右侧足背动脉血流量和左/右侧足背动脉内径水平随之减小,脂蛋白a随之升高(P<0.05);观察组患者治疗前后脂蛋白及足背动脉血流相关指标水平出现变化,治疗后脂蛋白a下降,左/右侧足背动脉血流量及其足背动脉内径水平增大(P<0.05);多因素Logistic回归分析结果显示:足背动脉血流量、足背动脉内径、脂蛋白a水平和糖尿病病程是影响老年糖尿病足发生的独立危险因素(P<0.001)。结论不同Wagner分级糖尿病足足背动脉血流量和脂蛋白a指标水平存在差异,通过检测其指标水平,可为老年糖尿病足的临床诊断、治疗效果评估及制定治疗方案提供可靠资料。
Objective This article aims to analyze the changes and its clinical significance in blood flow of arteria dorsalis pedis and lipoprotein a level of patients with senilediabetic foot and to provide clinical analysis for the treatment of the disease.Methods 204 elderly patients with diabetic foot who were hospitalized in Western Theater General Hospital from Au-gust 2017 to August 2020 were selected as the observation group.In addition,200 patients with type 2 diabetes who did not have diabetic foot in the long-term outpatient clinic in this hospital during the same period were selected as the control group.The clinical,imaging and laboratory data of patients in the two groups were collected,and blood flow of arteria dorsalis pedis and lipoprotein a level of patients in different groups,Wagner classification,and before and after treatment were compared.The multiple Logistic regression models was used to analyze the relevant factors affecting the occurrence of senile diabetic foot.Results There were no significant differences in diabetes biochemical indicators(TG,HDL-C,LDL-C,TC,FPG)and li-poprotein-related indicators(ApoA,Apo B,Apo E)between the control group and the observation group(P>0.05).The blood flow of the left/right arteria dorsalis pedis and the inner diameter of the left/right arteria dorsalis pedis of the observation group were significantly lower than those of the control group,while lipoprotein a level was higher than that in the control group(P<0.05).In the observation group,with the increasing of Wagner classification,the blood flow of left/right arteria dorsalis pedis and the inner diameter of left/right arteria dorsalis pedis decreased,and lipoprotein a increased(P<0.05).In.the observation group,the levels of lipoproteins and blood flow related indicators arteria dorsalis pedis of patients changed significantly before and after treatment,lipoprotein a decreased,and the blood flow in left/right darteria dorsalis pedis and the inner diameter of arteria dorsalis pedis increased(P<0.05).The results of
