摘要
心血管疾病是导致我国人群死亡及疾病负担的主要病因,冠心病的早期诊断是基于患者的临床症状、心电图以及生物标志物才得以实现的,其中生物标志物对于冠心病的诊断意义尤为重要。临床研究发现,开展以病证结合的中西医治疗模式,在提高对冠心病的诊断和临床疗效方面具有显著作用。中医流行病学调查发现,以痰浊、血瘀为证素的证候类型最为多见,其中痰瘀互结证冠心病是中医临床常见的辨证分型,因此开展冠心病痰瘀相关的证候本质研究,找到证候的特异性标志物,并作为微观辨证依据,成为目前中医药防治冠心病研究的焦点。本文从临床生化指标、蛋白组学技术、基因组学技术、代谢组学技术等方面做出梳理与总结,旨在为发现冠心病痰瘀相关特异性生物标志物,并用以早期临床诊断提出思考。
Cardiovascular disease is the main cause of death and disease burden in Chinese population.The early diagnosis of coronary heart disease(CHD)is based on the patient's clinical symptoms,electrocardiogram,and biomarkers,and biomarkers for CHD are particularly important.Clinical studies have found that the treatment model of combination of Chinese and Western medicine with disease and syndrome can improve the diagnosis and clinical efficacy of coronary heart disease.The epidemiological survey of Chinese medicine found that the phlegm and blood stasis are the most common syndrome factors,and the phlegm and blood stasis syndrome is one of the most common syndrome types.The study of the nature of syndromes,the identification of specific markers of syndromes,and microscopic basis,has become the focus of current research on prevention and treatment of CHD with phlegm and blood-stasis syndrome.This article summarizes the study of markers in clinical biochemical indicators,proteomics,genomics,metabolomics,aiming to find out the specific biomarkers related to phlegm and blood-stasis syndrome in CHD,and to apply them on early clinical diagnosis.
