摘要
目的:探讨甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原199(CA199)及癌抗原50(CA50)联合检测在原发性肝癌鉴别诊断中的应用价值。方法:选取2018年1月至2019年12月我院收治的肝癌患者、肝病患者(肝硬化、慢性肝炎)以及同期健康体检者共195例作为研究对象,分为肝癌组、肝病组与对照组,每组各65例。比较三组人群血清AFP、CEA、CA50及CA199水平,分析AFP、CEA、CA50及CA199检测肝癌阳性率,采用受试者工作特征特性曲线(ROC)下面积(AUC)评价AFP、CEA、CA50、CA199单独及联合检测肝癌的诊断价值。结果:肝癌组与肝病组患者AFP、CEA、CA50及CA199水平明显高于对照组(P<0.05);肝癌组患者AFP、CEA、CA50及CA199水平明显高于肝病组(P<0.05)。AFP、CEA、CA50及CA199指标检测肝癌的阳性率分别为72.31%、70.77%、72.31%、75.38%。AFP、CEA、CA50及CA199单独及联合检测AUC值分别为0.851、0.810、0.760、0.739、0.944。结论:血清AFP、CEA、CA50及CA199指标联合检测有助于提高原发性肝癌检出准确性,对早期鉴别诊断肝癌具有较高的诊断价值。
Objective:To explore the application value of combined detection of alpha-fetoprotein(AFP),carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199)and cancer antigen 50(CA50)in the differential diagnosis of liver cancer.Methods:From Jan.2018 to Dec.2019,195 patients with liver cancer,liver disease(cirrhosis,chronic hepatitis)and healthy people in our hospital during the same period were selected as the research objects,and they were divided into liver cancer group,liver disease group and control group,with 65 cases in each group.The serum levels of AFP,CEA,CA50 and CA199 were compared among the three groups,and the positive rates of AFP,CEA,CA50 and CA199 in the detection of HCC were analyzed.The area under receiver operating characteristic curve(ROC)was used to evaluate the diagnostic value of AFP,CEA,CA50 and CA199 alone or in combination.Results:The levels of AFP,CEA,CA50 and CA199 in liver cancer group and liver disease group were significantly higher than those in control group(P<0.05);the levels of AFP,CEA,CA50 and CA199 in liver cancer group were significantly higher than those in liver disease group(P<0.05).The positive rates of AFP,CEA,CA50 and CA199 were 72.31%,70.77%,72.31% and 75.38% respectively.The AUC values of AFP,CEA,CA50 and CA199 were 0.851,0.810,0.760,0.739 and 0.944,respectively.Conclusion:Combined detection of serum AFP,CEA,CA50 and CA199 is helpful to improve the detection accuracy of primary liver cancer,and has high diagnostic value for early differential diagnosis of liver cancer.
