摘要
目的:探讨小脑伴有皮层静脉引流的硬脑膜动静脉瘘的MRI表现,尤其是DWI、T2*WI表现。方法:回顾性分析5例DSA证实的小脑伴有皮层静脉引流的硬脑膜动静脉瘘病例MRI资料,分析其发病部位以及MRI信号特征,尤其是DWI、T2*WI表现。结果:4例双侧小脑均累及,其中1例同时累及双侧额顶枕叶及丘脑;1例累及右侧小脑及小脑中脚、桥脑、中脑。所有病例均见脑实质区明显增多增粗的细小流空血管影,单侧为主。3例脑实质钙化、含铁血黄素沉积呈T1WI高低混杂信号,T2WI、FLAIR低信号,T2*WI片状明显低信号,无占位效应。2例亚急性梗塞呈DWI高信号,ADC图高信号。3D TOF MRA可见静脉窦区异常高信号。结论:小脑伴有皮层静脉引流的硬脑膜动静脉瘘MRI表现较具特征性,MRI有助于DSA瘘口评估前的诊断及脑实质损害情况的评估。
Objective:To investigate the MRI findings of cerebellar dural arteriovenous fistula(DAVF)with cortical venous drainage,especially the features of DWI and T2*WI.Methods:Retrospectively,five cases of MRI data of cerebellar DAVF with cortical venous drainage confirmed by DSA,were analyzed,focusing on the location and signal intensity characteristics,especially the feature of DWI and T2*WI.Results:4 cases of them,bilateral cerebellum were involved,including one case involving the bilateral frontal,parietal and occipital lobe and thalamus;1 cases involving the right cerebellum and cerebellar peduncles,the pons,and midbrain.All cases were seen significantly increased size of the small flow void vascular,unilateral mainly.Three cases of them with calcification,hemosiderin deposition showed mixed-signal on T1 WI,T2 WI and FLAIR,while low signal on T2*WI.Two cases with subacute infarction showed DWI hyperintensity and ADC map high signal.There was abnormally high signal on lateral sinus on 3 D TOF MRA.Conclusion:The typical MRI findings of cerebellar DAVF with cortical venous drainage contribute to the diagnosis and the assessment of brain parenchymal damage of them prior to the assessment of the DSA.
