摘要
目的:探讨磁共振(magnetic resonance imaging,MRI)三维双回波稳态水激发(three-dimensional dua-echo steady-state with water-exit,3D DESS-WE)序列在面神经炎诊断中的应用价值。方法:回顾2021年1月至3月例经过临床诊断的22例外周性面神经炎患者的MRI检查资料,包含结合64通道科研型头颈联合线圈(HN64)对双侧面神经颅外段进行的3D DESS-WE序列扫描;统计患侧面神经腮腺段主干、一级及二级分支显示良好的例数;测量患侧、健侧面神经主干横截面积及信号强度,计算面积比值Sr及信号强度比值SIr,作单样本均数的t检验;统计面神经主干及一、二级分支受累总数及各类型分布。结果:22例检查中,面神经主干、二支一级分支及五支二级分支显示良好支数分别为22、20、19、18、16、15、10、8支;患侧面神经增粗(Sr=1.67±0.62,t=2.81,P<0.05),信号强度增高(SIr=1.42±0.58,t=2.96,P<0.05);病变累及面神经主干及一、二级分支分别有16、14、8例。结论:3D DESS-WE序列在显示面神经颅外段及诊断面神经炎中有较大价值。
Objective:The purpose of this study was to explore the value of MRI 3 D DESS-WE sequence in the diagnosis of facial neuritis.Methods:The MRI data of 22 patients with peripheral facial neuritis diagnosed clinically from January 2021 to March were reviewed,including images of bilateral extracranial facial nerves on 3 D DESS-WE sequence with 64 channels scientific head-neck coil(HN64).The number of the trunk,primary and secondary branches of the affected lateral nerves showed good were counted.The cross-sectional area and signal intensity of bilateral nerves was measured,t test of mean of single samplewas used to compare the area ratio(Sr)and signal intensity ratio(SIr).The number of affected facial nerve trunk and primary and secondary branches was counted respectively.Results:In 22 cases of examination,the numbers of the facial nerve trunk,two primary branches and five secondary branches showed good were 22,20,19,18,16,15,10,and 8,respectively.The diseased nerves were thickened(Sr=1.67±0.62,t=2.81,P<0.05)and signal intensity increased(SIr=1.42±0.58,t=2.96,P<0.05).The lesions involved the main facial nerve and the primary and secondary branches in 16,14,and 8 cases respectively.Conclusion:3 D DESS-WE sequence is of great value in displaying the extracranial segment of facial nerve and diagnosing facial neuritis.
