摘要
目的观察中医综合康复治疗膝关节骨性关节炎的疗效及其对骨关节功能和骨质的影响。方法选取2015年1月至2020年6月在上海市嘉定区南翔医院诊治的膝关节骨性关节炎患者104例,将其分为观察组和对照组,各52例。对照组给予常规的西药治疗,观察组在对照组的基础上予以中医综合康复治疗。观察两组治疗疗效和不良反应发生率,比较两组治疗前后中医证候评分、国际膝关节文献委员会膝关节评估表(IKDC)评分、Tegner评分、股四头肌萎缩程度、平衡能力、步态、血清软骨寡聚基质蛋白(COMP)、骨钙素(BGP)、骨保护素(OPG)和Ⅱ型胶原C-端肽(CTX-Ⅱ)水平的变化。结果观察组总有效率明显高于对照组,差异有统计学意义(P<0.05),而两组不良反应发生率比较,差异无统计学意义(P>0.05)。治疗后两组的中医证候评分、股四头肌萎缩程度、前后轴指数、左右轴指数、COMP和CTX-Ⅱ水平均较治疗前明显降低(P<0.01),IKDC评分、Tegner评分、步长、步速、BGP和OPG水平均较治疗前明显升高(P<0.01),且观察组与对照组比较降低或升高更明显,差异均有统计学意义(P<0.01)。结论中医综合康复治疗膝关节骨性关节炎的疗效显著,能够明显改善膝关节功能和骨质代谢平衡。
Objective To observe the effect of comprehensive rehabilitation of traditional Chinese medicine(TCM)in patients with knee joint osteoarthritis and its influence on the joint function and bone metabolism.Methods A total of 104 patients with osteoarthritis of the knee treated in Nanxiang Hospital of Jiading District from January 2015 to June 2020 were divided into the observation group and the control group,with 52 cases in each group.The control group was given conventional western medicine treatment,and the observation group was given TCM comprehensive rehabilitation treatment on the basis of the control group.The therapeutic efficacy and the incidence of adverse reactions in the two groups were observed.The changes of TCM syndrome score,IKDC score,Tegner score,quadriceps atrophy degree,balance ability,gait,serum cartilage oligomyelic matrix protein(COMP),bone gla-protein(BGP),osteoprotectin(OPG)and C-telopeptide of typeⅡcollagen(CTX-Ⅱ)in the two groups before and after treatment were compared.Results The total effective rate of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05),but there was no significant difference on the incidence of adverse reactions between the two groups(P>0.05).After treatment,TCM syndrome score,quadriceps atrophy degree,front and rear axis index,left and right axis index,COMP and CTX-Ⅱlevels significantly decreased(P<0.01),IKDC score,Tegner score,step length,pace,BGP and OPG levels significantly increased(P<0.01),and the observation group decreased or increased more significantly than the control group(P<0.01).Conclusion Comprehensive rehabilitation of TCM is effective in treating knee osteoarthritis,which could significantly improve knee joint function and bone metabolism balance.
