14例急性淋巴细胞白血病患儿停疗后体重指数变化及临床资料分析 被引量:1

摘要 目的本文回顾性分析化疗结束后无病生存的ALL患儿的体重指数及临床资料,初步探讨治疗后无病生存儿童超重与相关因素。方法研究人群包括所有在我院接受过化疗的2-14岁急性淋巴细胞白血病儿童。每位患儿在诊断时、诊断后35d、第1年、第2年内收集BMI z评分。并收集相关时间段血糖、血脂指标,计算化疗结束时的常用化疗药物的累积量。利用IBM SPSS25软件进行数据统计分析。结果无病生存儿童男孩多于女孩,总体体重BMI z值呈逐年上升趋势。无病生存儿童超重与确诊时体重BMI z值、治疗35d及治疗1年时的BMI z值密切相关。超重儿血糖及血脂指标略高于正常体重儿。血糖水平及高密度脂蛋白水平在两组之间有显著性差异。超重儿与正常儿互相比较发现,随着地塞米松、长春新碱、蒽环类、巯基嘌呤等药物的累积量的增加,超重发生率增加。差异有统计学意义。结论诊断时及治疗35d,第1年体重增加与长期BMI的关系要比早期体重增加更密切,与药物的累积量有一定的关系。这些结果可能有助于患儿早期干预体重增加,预防肥胖症发生及代谢综合征发生,从而提高长期无病生存白血病患儿的生存质量。 Objective To retrospectively analyze changes in body mass index(BMI)and clinical data of acute lymphoblastic leukemia(ALL)children with disease-free survival after chemotherapy,so as to discuss the overweight and related factors in disease-free survival children after treatment.Methods All 2-14 years old ALL patients received chemotherapy at the First Affiliated Hospital of Xinjiang Medical University were included in our study.BMI z scores were collected at the time of diagnosis,35 days after diagnosis,first year,and 2 years.Blood glucose,blood lipid indicators in the relevant time period,and the accumulation of commonly used chemotherapy drugs at the end of chemotherapy were also collected.IBM SPSS software 25 were used for statistical data analysis.Results There are more boys than girls in disease-free survival children.The overall body weight BMI z value increased year by year.After treatment,the overweight of the child was closely related to the BMI z value at the time of diagnosis,and the BMI z value at 35 days after treatment and 1 year after treatment.The blood sugar and blood lipid indexes of overweight children were slightly higher than those of normal weight children.Blood glucose levels and high-density lipoprotein levels were significantly different between the two groups.After comparation of overweight and normal children,we found that the incidence of overweight increased with the accumulation of drugs such as dexamethasone,vincristine,anthracyclines,and thiopurine.The difference was statistically significant.Conclusions Our data indicate that at the time of diagnosis and 35 days after treatment,the relationship between weight gain in the first year and long-term BMI is more closely related to early weight gain.There is a certain relationship with the amount of drug accumulated.These results may help children to early intervention in weight gain,prevent obesity and metabolic syndrome,so as to achieve the long-term disease-free survival of children with leukemia after treatment.
