摘要
目的探讨乳腺癌新辅助化疗患者血清长链非编码RNA(LncRNA HOTAIR)及同源异型基因簇A远端转录本(HOTTIP)的表达以及在化疗疗效预测中的价值。方法前瞻性选取在苏州大学附属张家港医院行新辅助化疗的46例乳腺癌患者作为研究组,另外收集40例同院健康体检的正常人群作为对照组。采用反转录-实时定量聚合酶链式反应检测2组研究对象血清中LncRNA HOTAIR及HOTTIP的表达,并分析研究组中LncRNA HOTAIR及HOTTIP的表达与年龄、肿瘤大小、淋巴结情况、TMN分期,激素受体表达、HER-2表达的关系,并在3~4个化疗周期后评估其化疗疗效,采用受试者工作特征(ROC)曲线对LncRNA HOTAIR及HOTTIP的疗效预测效能进行评价。结果研究组血清LncRNA HOTAIR、HOTTIP的表达水平(2.58±1.32、3.52±1.50)较对照组表达水平(1.59±0.89、1.91±1.33)均显著增高,差异均有统计学意义(P<0.05)。高表达的外周血清乳腺癌的HOTAIR与肿瘤大小相关(P<0.05),与年龄、淋巴结是否转移、TNM分期、激素受体表达、HER-2表达无明显相关性(P>0.05);血清高表达HOTTIP与肿瘤分期有关(P<0.05),与年龄、肿瘤大小、淋巴结是否转移、激素受体表达、HER-2表达无明显相关性(P>0.05)。乳腺癌患者新辅助化疗后,化疗有效组22例,无效组24例,有效率47.8%。化疗有效组患者血清HOTAIR表达水平(2.18±1.32)明显低于化疗无效组(2.94±1.23),差异有统计学意义(P<0.05),而化疗有效组(3.10±1.46)与化疗无效组(3.90±1.47)患者的血清HOTTIP表达差异无统计学意义(P>0.05)。血清LncRNA HOTAIR单独预测化疗疗效的AUC、敏感度和特异度分别为0.688、59.1%、79.2%,血清LncRNA HOTTIP单独预测化疗疗效的AUC、敏感度和特异度分别为0.641、72.7%、70.8%,2者联合后的AUC、敏感度和特异度分别为0.708、60.5%、87.5%。结论血清LncRNA HOTAIR和HOTTIP在乳腺癌患者中高表达,2者联合可以作为乳腺癌新辅助化疗疗效的潜在
Objective To investigate the expressions of serum LncRNA HOTAIR and HOTTIP in patients with neoadjuvant chemotherapy for breast cancer and its value in predicting the efficacy.Methods A total of 46 patients with breast cancer who underwent neoadjuvant chemotherapy in our hospital were collected as the study group,and serum samples from another 40 healthy people in Zhangjiagang Hospital Affiliated to Suzhou University were collected as a control group,which were detected by quantitative reverse transcription polymerase chain reaction.The expressions of LncRNA HOTAIR and HOTTIP in serum were analyzed for their relationship with age,tumor size,lymph node status,TMN stage,hormone receptor expression,and HER-2 expression,and their efficacy was evaluated after 3-4 chemotherapy cycles.The receiver operating characteristic(ROC)curve was used to evaluate the efficacy of LncRNA HOTAIR and HOTTIP.Results The expression levels of serum LncRNA HOTAIR and LncRNA HOTTIP in the study group(2.58±1.32,3.52±1.50)were significantly higher than those in the control group(1.59±0.89,1.91±1.33),and the differences were statistically significant(P<0.05).HOTAIR of breast cancer with high expression in peripheral serum was related to tumor size(P<0.05),and there was no significant correlation with age,lymph node metastasis,TNM stage,hormone receptor expression,and HER-2 expression(P>0.05).HOTTIP of breast cancer with high expression in peripheral serum was related to tumor stage(P<0.05),and there was no significant correlation with age,tumor size,lymph node metastasis,hormone receptor expression,and HER-2 expression(P>0.05).After neoadjuvant chemotherapy,22 patients in the effective group and 24 patients in the ineffective group,with an effective rate of 47.8%.The level of serum HOTAIR expression in the chemotherapy effective group(2.18±1.32)was significantly lower than the chemotherapy ineffective group(2.94±1.23),the difference was statistically significant(P<0.05),while there was no significant difference in level of serum HOTTIP
