摘要
目的观察体外冲击波疗法治疗颞下颌关节紊乱病的临床效果。方法选取2019年1月至2020年5月就诊于莆田市第一医院疼痛科的颞下颌关节紊乱病患者,共30例,均给予体外冲击波治疗,1次/5 d,共4次。采用颞下颌关节功能评价(Friction指数法)、视觉疼痛模拟评分法(VAS)、最大张口度、SF-36量表对患者的疗效进行评估。结果第1次治疗后、第4次治疗后、第4次治疗后1个月,颞下颌关节功能评价和VAS疼痛评分均较治疗前明显下降(P<0.01),最大开口度及SF-36评分较治疗前明显改善(P<0.01),并且各时间点差异具有统计学意义(P<0.01)。结论冲击波治疗颞下颌关节紊乱病具有显著的临床效果,治疗效果随治疗次数的增多而出现时间依赖性及累积效应,在疗程结束1月后随访症状未出现反复,且进一步缓解,值得更多地临床应用。
Objective To observe the clinical effect of extracorporeal shock waves in the treatment of temporomandibular joint disorders.Methods A total of 30 patients with temporomandibular joint disorders admitted to the department of Pain in the First Hospital of Putian from January 2019 to May 2020 were selected.All patients were given extracorporeal shock waves,once every 5 days for 4 times.Temporomandibular joint function evaluation(Friction index method),visual pain simulation score(VAS),maximum mouth opening,and SF-36 scale were used to evaluate the efficacy of treatment.Results After the first treatment,after the fourth treatment,and 1 month after the fourth treatment,the temporomandibular joint function evaluation and the VAS scores were significantly lower than those before treatment(P<0.01),the maximun opening and SF-36 scores were significantly improved compared with those before treatment(P<0.01),and the differences at each time point were ststistically significant(P<0.01).Conclusion Extracorporeal shock waves has a significant clinical effect on the treatment of temporomandibular joint disorders.With the increase of treatment times,the therapeutic effect appeared time dependence and cumulative effect,and at the follow-up 1 month after the end of the course,the symptoms did not relapse and were further relieved.It is worth more clinical application.
