摘要
隔物灸能够有效改善类风湿关节炎(rheumatoid arthritis,RA)患者的临床症状,延缓病程进展。实验研究表明,隔物灸可调节RA相关的细胞因子、免疫球蛋白M(immunoglobulinM,IgM)、肿瘤坏死因子-α(tumor necrosisfactor-α,TNF-α),纠正免疫功能,减少免疫系统疾病的发病率。相较于单纯艾灸,隔物灸可通过辐射作用、温热作用刺激穴位,同时增强了药物的透皮吸收能力,产生穴、药、灸联合效应。但目前对于隔物灸治疗RA的作用机制研究缺乏,基础研究薄弱,如药物经穴吸收对穴位产生的刺激作用、经穴吸收药物如何转运及代谢、艾灸如何与药物配合以发挥其最大功效等问题亟待解决,而基础研究的缺失导致用药规律不明确,组方配药缺乏科学性。故需继续加强隔物灸治疗RA的基础研究,梳理“穴-灸-药”三维作用,根据性味、归经找出隔物灸临床用药的内在规律,同时重视应用研究,积极改良剂型,研制新产品,以更好地服务于临床。
Sandwiched moxibustion can effectively improve the clinical symptoms of patients with rheumatoid arthritis(RA)and delay the progress of the disease course.Experimental studies show that separated moxibustion can regulate RA related cytokines,immunoglobulin M(IgM)and tumor necrosis factor-α(TNF-α),correct immune function and reduce the incidence rate of immune system diseases.Compared with simple moxibustion,sandwiched moxibustion stimulates acupoints through radiation and warming,enhances the transdermal absorption of drugs,and produces the combined effect of acupoints,drugs and moxibustion.However,at present,there is a lack of research on the action mechanism of sandwiched moxibustion,and the basic research is weak,such as the stimulating effect of drug acupoint absorption on acupoints,how to transport and metabolize drugs absorbed by acupoints,and how moxibustion cooperates with drugs to give full play to its maximum efficacy.The lack of basic research leads to the unclear drug law of sandwiched moxibustion and the lack of scientificity of making up prescriptions.Therefore,it is necessary to continue to strengthen basic research,sort out the three-dimensional role of"acupoint-moxibustion-medicine",find out the internal law of clinical medication of sandwiched moxibustion according to nature,flavour and meridian tropism,pay attention to application research,actively improve dosage forms and develop new products,so as to better serve the clinic.
