摘要
目的探讨简明损伤定级标准(abbreviated injury scale,AIS)-损伤严重程度(injury severity score,ISS)评分系统在车祸伤患者外科急诊中的应用及与凝血功能的关系。方法选择2020年3月—2021年4月车祸伤外科急诊患者76例作为对象,均给予常规急诊救治,并对患者进行追踪随访,根据患者预后分为死亡组和存活组。采用AIS-ISS量表对患者进行评估,根据评分结果分为轻型组(ISS评分<17分)和重型组(ISS评分≥17分)。采用全自动凝血仪(型号TOP500)测定患者纤维蛋白原(FIB)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)水平,并与AIS-ISS评分进行相关性分析。结果死亡组PT、APTT、TT水平均高于存活组(P<0.05);FIB水平均低于存活组(P<0.05);ISS评分<17分50例,≥17分26例;重型组PT、APTT、TT水平均高于轻型组(P<0.05);FIB水平均低于轻型组(P<0.05);Pearson相关性分析结果表明:车祸伤患者AIS-ISS评分与PT、APTT、TT水平呈正相关性(P<0.05);与FIB水平呈负相关性(P<0.05)。结论AIS-ISS评分系统用于车祸伤患者外科急诊中效果理想,能反映患者的疾病严重程度,且与患者凝血功能存在紧密联系,可指导临床治疗。
Objective To explore the application of the concise injury rating standard(AIS)-injury severity(ISS)scoring system in the emergency surgery of traffic accident patients and its relationship with coagulation function.Methods A total of 76 patients in the emergency department of trauma surgery in traffic accidents from March 2020 to April 2021 were selected as subjects.All patients were given routine emergency treatment and followed up.According to the prognosis of patients,they were divided into death group and survival group.The AIS-ISS scale was used to evaluate the patients,and they were divided into light group(ISS score<17 points)and severe group(ISS score≥17 points)according to the scoring results.Use an automatic coagulometer(model TOP500)to determine the patient's fibrinogen(FIB),activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT)levels,and score with AIS-ISS Perform correlation analysis.Results The levels of PT,APTT,and TT in the death group were higher than those in the survival group(P<0.05);FIB levels were lower than those in the survival group(P<0.05);50 cases with ISS score<17 points,26 cases with≥17 points;severe group the levels of PT,APTT,and TT were higher than those of the light group(P<0.05);the FIB level was lower than that of the light group(P<0.05);the results of Pearson correlation analysis showed that the AIS-ISS score of traffic accident patients was correlated with PT,APTT,TT The level was positively correlated(P<0.05);and the FIB level was negatively correlated(P<0.05).Conclusion The AIS-ISS scoring system has an ideal effect in emergency surgery for patients with traffic accidents.It can reflect the severity of the patient's disease and is closely related to the patient's coagulation function,which can guide clinical treatment.
