摘要
目的探讨伴肠母细胞分化的胃腺癌(gastric adenocarcinoma with enteroblastic differentiation,GAED)合并神经内分泌小细胞癌的临床病理特征、免疫表型及预后。方法应用HE染色、免疫组织化学染色对我院收集的1例伴肠母细胞分化的胃腺癌合并神经内分泌小细胞癌进行临床病理分析,并进行相关的国内外文献复习,探讨其临床病理特征及预后。结果胃大部切除标本镜下见肿瘤主要由两种成分组成:第一种成分占整个肿瘤的90%,主要由腺样、乳头状、筛状结构组成,部分呈原肠样;第二种成分占整个肿瘤的10%,小圆肿瘤细胞呈弥漫失粘附生长。弥漫生长区和腺样结构区相互交叉生长。免疫组织化学染色显示伴肠母细胞分化的腺癌成分阳性表达SALL4、MUC2、MUC6、CDX-2、villin、glypican 3、AFP,神经内分泌小细胞癌阳性表达Syn、CgA、CD56。结论伴肠母细胞分化的胃腺癌合并神经内分泌小细胞癌极其罕见,预后极差,应充分认识其临床病理特点,避免低估其恶性程度。
Objective To investigate the clinicopathological features,immunophenotypes and prognosis of gastric adenocarcinoma with enteroblastic differentiation(GAED)combined with neuroendocrine small cell carcinoma.Methods The sample of a case of GAED combined with neuroendocrine small cell carcinoma collected in our hospital was analyzed by HE and immunohistochemistry staining,and relative literature was reviewed to explore its clinicopathological features and prognosis.Results Microscopic features of the specimen:the tumor is mainly composed of two components.The first one accounts for 90%of the whole tumor,which is composed of glandular,papillary and sieve-like structures and partly is protointestinal-like.The second one accounts for 10%of the whole tumor and shows diffuse non-adhesive growth of small round tumor cells.The diffuse growth area and glandular structure area cross with each other.Immunohistochemistry:the GAED specimen shows SALL4,MUC2,MUC6,CDX-2,villin,Glypican-3 and AFP positive,and Syn,CgA and CD56 are positively expressed in the neuroendocrine small cell carcinoma tissue.Conclusion GAED combined with neuroendocrine small cell carcinoma is extremely rare with a poor prognosis.We should fully understand its pathological characteristics and avoid underestimating its malignant degree.
