摘要
目的分析诱导型一氧化氮合酶(iNOS)基因多态性与病毒性心肌炎(VMC)易感性和心肌病变程度的关系。方法选取2018年1月-2020年7月海南医学院第一附属医院收治的VMC患者65例为VMC组,并选取同期体检健康者48名为对照组。采用聚合酶链式反应-限制性内切酶片段长度多态性(PCR-RFLP)技术,检测iNOS基因rs2779248和rs1137933位点的基因型分布情况,酶联免疫吸附法(ELISA)检测血清iNOS水平,比较两组iNOS基因多态性及不同基因型血清iNOS水平。结果VMC组iNOS基因rs2779248位点T等位基因频率高于对照组(P<0.05),rs1137933位点TT基因型和T等位基因频率均高于对照组(P<0.05);不同基因型血清iNOS水平比较有统计学意义(P<0.05),且VMC组患者各基因型血清iNOS水平均高于对照组(P<0.05);急性期和重症VMC患者iNOS基因rs2779248和rs1137933位点TT基因型和T等位基因频率均高于恢复期和轻症患者(P<0.05)。结论iNOS基因rs2779248位点和rs1137933位点的多态性可能与VMC患者易感性及心肌病变程度有关。
OBJECTIVE To analyze the relationship between inducible nitric oxide synthase(iNOS)gene polymorphism and susceptibility to viral myocarditis(VMC)and severity of cardiomyopathy.METHODS A total of 65patients with VMC who were treated in the First Affiliated Hospital of Hainan Medical College from Jan 2018to Jul 2020were assigned as the VMC group,meanwhile,48healthy people who received physical examination were set as the control group.The genotypes at rs2779248and rs1137933loci of iNOS gene were detected by using polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),the serum iNOS level was detected by means of enzyme-linked immunosorbent assay(ELISA),and the iNOS gene polymorphism was compared between the two groups,and the serum iNOS level was compared among the patients with different genotypes.RESULTS The frequency of T allele at rs2779248locus of iNOS gene of the VMC group was significatly higher than that of the control group(P<0.05),the frequencies of TT genotype and T allele at rs1137933locus of the VMC group were significatly higher than those of the control group(P<0.05).There was significant difference in the serum iNOS level among the different genotypes(P<0.05),and the serum iNOS level of the different genotype was significatly higher in the VMC group than in the control group(P<0.05).The frequencies of TT genotype and T allele at rs2779248and rs1137933loci of iNOS gene of the patients with acute stage of VMC and severe VMC were significatly higher than those of the patients with recovery stage of VMC and mild VMC(P<0.05).CONCLUSIONThe polymorphisms at rs2779248and rs1137933loci of iNOS gene may be associated with the susceptibility to VMC and severity of cardiomyopathy.
