脑CT灌注成像在首发抑郁症患者诊断中的效果 被引量:5

Effect of cerebral CT perfusion imaging in the diagnosis of first episode depression

摘要 目的分析脑CT灌注成像在首发抑郁症患者中的诊断价值。方法选取本院2020年1月~2021年1月期间收治的180例首发抑郁症患者作为观察组,同时选取同期门诊体检的180例健康人员作为对照组,均进行脑CT灌注成像检查,比较两组平扫、增强扫描后颅脑各脑叶CT值。结果 (1)两组左顶叶、右顶叶、左颞叶、右颞叶、左枕叶、右枕叶平扫后CT值、TIP、MTT、rCBF、rCBV差异无统计学意义(P>0.05),观察组左额叶、右额叶平扫CT值均低于对照组(P<0.05)。(2)两组左顶叶、右顶叶、左颞叶、右颞叶、左枕叶、右枕叶增强扫描后CT值、TIP、MTT、rCBF、rCBV差异无统计学意义(P>0.05),观察组左额叶、右额叶增强扫描CT值均低于对照组(P<0.05)。结论首发抑郁症患者左右额叶脑组织密度CT值均低于正常对照,可能存在大脑额叶功能改变,对首发抑郁症诊断具有一定的临床价值。 Objective To analyze the diagnostic value of cerebral CT perfusion imaging in patients with first episode depression. Methods A total of 180 patients with first episode depression admitted to our hospital from January 2020 to January 2021 were selected as the observation group,and 180 cases of healthy people who underwent outpatient physical examination during the same period were selected as the control group. All patients underwent cerebral CT perfusion imaging examination,and the CT values of each lobe of the brain after unscanned and enhanced scanning between the two groups were compared. Results (1)There were no statistically significant differences in CT values,TIP,MTT,rCBF and rCBV of left parietal lobe,right parietal lobe,left temporal lobe,right temporal lobe,left occipital lobe and right occipital lobe between the two groups(P>0.05). The plain CT values of left frontal lobe and right frontal lobe in the observation group were lower than those in the control group(P<0.05).(2)There were no statistically significant differences in CT values,TIP,MTT,rCBF and rCBV of left parietal lobe,right parietal lobe,left temporal lobe,right temporal lobe,left occipital lobe and right occipital lobe after enhanced scanning between the two groups(P>0.05). The CT values of left frontal lobe and right frontal lobe in the observation group were lower than those in the control group(P<0.05). Conclusion The CT value of brain tissue density in left and right frontal lobes of patients with first episode depression is lower than that of normal people,which may have the change of brain function in frontal lobes,which has certain clinical value for the diagnosis of first episode depression.
