摘要
目的了解妊娠早期孕妇症状发生情况,识别妊娠早期症状群,并分析其影响因素,为妊娠早期孕妇的症状管理提供参考。方法2019年10月至2020年1月便利抽样选取某院门诊产检的妊娠早期孕妇227例为研究对象,采用记忆症状评估量表对进行问卷调查,采用探索性因子分析探索症状群,通过多重线性回归探讨症状群的影响因素。结果探索性因子分析得出,妊娠早期孕妇存在4个症状群,分别为精神心理症状群、活动无耐力症状群、消化道症状群、皮肤症状群;月收入、职业、居住地、孕周、孕前体质量指数(body mass index,BMI)、妊娠前运动情况是症状群的影响因素。结论妊娠早期孕妇会经历较多且复杂的症状,症状之间相互影响,以症状群的形式出现。建议临床护士全面评估妊娠早期孕妇的症状,开展以症状群为干预单位的症状管理,减轻症状对孕妇健康的影响。
Objective To explore the occurrence of symptoms among pregnant women in the first trimester,identify the symptom clusters in the first trimester pregnancy,and analyze influencing factors,so as to provide reference for symptom management among pregnant women in the first trimester.Methods Convenient sampling was used to select 227 pregnant women in the first trimester who received prenatal examination in the outpatient clinic of a hospital from October,2019 to January,2020.The Memorial Symptom Assessment Scale was used for investigation.Exploratory factor analysis was applied to identify symptom clusters,and multiple linear regression analysis was used to determine the factors affecting symptom clusters.Results Four symptom clusters of the first trimester pregnancy were identified by exploratory factor analysis,including mental and psychological symptom cluster,activity intolerance symptom cluster,gastrointestinal symptom cluster,and skin symptom cluster.Monthly income,occupation,residence,gestational age,BMI before pregnancy and exercise before pregnancy were influencing factors of the symptom clusters.Conclusions Pregnant women in the first trimester may experience many complicated symptoms,which affect each other and appear in the form of symptom clusters.It is suggested that clinical nurses should comprehensively assess the symptoms of pregnant women in the first trimester,and provide cluster-based symptom management,so as to alleviate the effects of the symptoms on the health of pregnant women.
