摘要
目的探讨美托洛尔治疗慢性心力衰竭(CHF)患者血药浓度与心功能恢复的相关性。方法选择2018年1月至2020年12月南通大学附属海安医院100例拟应用美托洛尔治疗的CHF患者,统计患者美托洛尔剂量达到最大耐受剂量持续1周后的血药浓度,评估患者心功能恢复情况,比较心功能恢复与未恢复患者的基线资料和血药浓度。结果治疗3个月后,评估心功能恢复情况发现,患者心功能恢复28例(28.00%),患者心功能未恢复72例(72.00%);心功能未恢复患者血药浓度低于恢复患者,差异有统计学意义(P<0.05);其余资料比较,差异无统计学意义(P>0.05);经Logistic回归分析结果显示,男性、血药浓度低可能与CHF患者美托洛尔治疗后心功能未恢复有关,是心功能未恢复的影响因子(OR>1,P<0.05)。结论不同CHF患者服用美托洛尔后血药浓度不一,治疗后得到的心功能恢复效果各异,血药浓度低会影响美托洛尔治疗CHF患者心功能恢复。
Objective To investigate the correlation between blood drug level of metoprolol and heart function recovery in treatment of chronic heart failure(CHF).Methods One hundred patients with CHF who were treated by metoprolol in our hospital from January 2018 to December 2020 were enrolled in the study.The blood drug level of metoprolol after reaching the maximum tolerated dose for one week were detected,and the recovery of cardiac function was evaluated,and the baseline data and plasma concentration in patients with cardiac function recovery were observed and compared with those of patients without cardiac function recovery.Results After 3-month treatment,28 patients(28.00%)had cardiac function recovery,but 72 patients(72.00%)had no cardiac function recovery.The blood drug levels of metoprolol in patients without cardiac function recovery were significantly lower than those of patients with cardiac function recovery(P<0.05).The Logistic regression analysis showed that male and low blood drug level of metoprolol might be related to the failure of cardiac function recovery in patients with CHF after metoprolol treatment,which were the influencing factor of failur of cardiac function recovery(OR>1,P<0.05).Conclusion The blood drug levels of metoprolol are different in different patients with CHF after metoprolol treatment,and the low blood drug levels of metoprolol will affect the cardiac function recovery of patients with CHF.
