摘要
肾着病为寒湿之邪附着于腰部经脉、留著而不祛所致。带脉起于季胁,回身一周,与肾着病发病部位一致,均在腰腹部。带脉病表现为腰腹重坠冷痛,与肾着病症状相似。以此推测,张仲景所言之肾着病当为带脉病。对于肾着病,张仲景强调缓补脾以实内,缓渗湿以祛邪,采用甘姜苓术汤治疗,甘姜苓术汤重用茯苓,增强温脾祛湿之效。甘姜苓术汤中干姜、白术分别为四两、二两,用量较大,可温散寒湿之邪。全方温而不滞,行气而不耗气,缓祛寒湿之邪。肾着病与带脉病为本虚标实之证,治疗当兼顾本虚,慎用攻伐之品。通过对肾着病证治思路的挖掘,可为带脉病的治疗提供思路。
Kidney affection by cold-dampness is caused by the evil of cold and dampness attached to the waist meridian and retained without elimination.The belt vessel starts from the quarter flank and turns around the whole body,which is consistent with the incidence of kidney affection by cold-dampness,both in the waist and abdomen.Belt vessel disease is characterized by heavy falling cold pain in the waist and abdomen,which is similar to the symptoms of kidney affection by cold-dampness.It is speculated that kidney affection by cold-dampness mentioned by Zhang Zhong-jing should be belt vessel disease.In terms of kidney affection by cold-dampness,Zhang Zhong-jing emphasizes that it is necessary to slowly replenish the spleen to consolidate the interior and slowly drain dampness to dispel evils.Licorice and Rhizoma Zingiberis and Poria and Atractylodes Macrocephala Decoction(LRZPAMD)is often used,which is Center-Regulating Pill removing Renshen(Ginseng)plus Fuling(Poria),and has the effect of warming the spleen and removing dampness.The dosage of Ganjiang(Rhizoma Zingiberis)and Baizhu(Atractylodes Macrocephala)is 4 liang and 2 liang,which can warm and disperse the evil of cold and dampness.The whole formula is warm but not stagnant.It can move qi without consuming qi,and slowly remove the evil of cold and dampness.Both kidney affection by cold-dampness and belt vessel disease have the syndrome of deficient root cause and excessive manifestations,so the treatment should take into account the deficiency of root cause and be careful with attacking products.Through the excavation of the treatment ideas of kidney affection by cold-dampness,the study can provide ideas for the treatment of belt vessel.
