摘要
目的探讨在早期应用大剂量丙种球蛋白治疗新生儿溶血症的价值。方法选取2018年1月至2019年5月本院收治的61例新生儿溶血症患儿,采用平衡序贯法分为对照组(常规治疗,n=29)和观察组(在对照组基础上联合大剂量丙种球蛋白治疗,n=32)。比较两组临床疗效、黄疸出现时间、黄疸消失时间、光疗时间、住院时间、治疗前后血清总胆红素及血红蛋白水平。结果观察组治疗总有效率为96.8%,明显高于对照组的79.3%,差异有统计学意义(P<0.05)。治疗后24、48、72 h,观察组黄疸出现时间、黄疸消失时间、光疗时间、住院时间均短于对照组(P<0.05)。治疗后,观察组血清总胆红素水平低于对照组,血红蛋白水平高于对照组(P<0.05)。结论早期应用大剂量丙种球蛋白治疗新生儿溶血症效果显著,可缩短临床各指标时间,改善血清总胆红素以及血红蛋白水平,值得临床推广运用。
Objective To investigate the value of early application of high dose of gamma globulin in the treatment of neonatal hemolysis.Methods From January 2018 to May 2019,61 children with neonatal hemolysis were selected and divided into control group(conventional treatment,n=29)and observation group(combined with high-dose gamma globulin treatment,n=32)by balanced sequential method.The clinical efficacy,jaundice occurrence time,jaundice disappearance time,phototherapy time,hospital stay,serum total bilirubin and hemoglobin levels before and after treatment were compared between the two groups.Results The total effective rate in the observation group was 96.8%,which was significantly higher than 79.3%in the control group,and the difference was statistically significant(P<0.05).24,48,72 h after treatment,jaundice occurrence time,jaundice disappearance time,phototherapy time and hospital stay in the observation group were shorter than those in the control group(P<0.05).After treatment,the serum total bilirubin level in observation group was lower than that in control group,and the serum hemoglobin level was higher than that in control group(P<0.05).Conclusion Early application of high dose of gamma globulin in the treatment of neonatal hemolysis is effective,can shorten the time of clinical indicators,improve serum total bilirubin and hemoglobin levels,it is worthly of clinical application.
