摘要
目的分析孕前行腹腔镜下子宫峡部环扎术在治疗宫颈机能不全中的应用价值。方法回顾性分析2016年1月—2019年5月科室收治的行孕前腹腔镜下子宫峡部环扎术(研究组)或孕期行预防性经阴道McDonald宫颈环扎术(对照组)的宫颈机能不全患者的临床病历资料及后续随访。结果研究组住院时间明显短于对照组(P<0.05),研究组患者的妊娠结局明显优于对照组(P<0.05)。结论对于宫颈机能不全的患者来说,行孕前的腹腔镜下子宫峡部环扎术可大大提高其足月妊娠率、降低流产率、早产率,且术中出血少、住院时间短、手术并发症少,在临床治疗中是安全、可行的。
Objective To analyze the application value of laparoscopic uterine isthmus cerclage in the treatment of cervical insufficiency before pregnancy.Methods Retrospective analysis was performed on the clinical data and follow-up of patients with cervical insufficiency who underwent pre-pregnancy laparoscopic uterine isthmus cerclage(study group)or prophylactic transvaginal McDonald cervical cerclage(control group)during pregnancy and were admitted to our department from January 2016 to May 2019.Results The hospitalization time of the study group was significantly shorter than that of the control group(P<0.05),and the pregnancy outcome of the study group was significantly better than that of the control group(P<0.05).Conclusion For patients with cervical insufficiency,laparoscopic uterine isthmus cerclage can significantly improve the rate of full-term pregnancy,reduce the rate of miscarriage and premature delivery,and it is safe and feasible in clinical treatment with less intraoperative bleeding,shorter hospitalization and fewer surgical complications.
