胫骨创伤性骨折患者术后创伤性关节炎发生情况及其影响因素 被引量:2

Occurrence and Influencing Factors of Postoperative Traumatic Arthritis in Patients with Tibial Traumatic Fracture

摘要 目的分析胫骨创伤性骨折患者术后创伤性关节炎的发生情况与影响因素,为临床干预提供指导。方法回顾性分析2019年2月至2020年2月我院收治的52例胫骨创伤性骨折患者的临床资料,均采用内固定术治疗。术后随访6个月,记录创伤性关节炎发生情况,并分析影响胫骨创伤性骨折患者术后发生创伤性关节炎的因素。结果52例胫骨创伤性骨折患者中,20例(38.46%)患者术后发生创伤性关节炎。经单因素与多因素分析显示,高能量损伤、Ⅳ~Ⅵ型骨折、骨质疏松、术后锻炼不依从为胫骨创伤性骨折患者术后发生创伤性关节炎的影响因素(OR>1,P<0.05)。结论高能量损伤、Ⅳ~Ⅵ型骨折、骨质疏松、术后锻炼不依从可导致胫骨创伤性骨折患者术后发生创伤性关节炎,需采取针对性预防措施,以降低创伤性关节炎发生率。 Objective To analyze the occurrence and influencing factors of postoperative traumatic arthritis in patients with tibial traumatic fracture,so as to provide guidance for clinical intervention.Methods The clinical data of 52 patients with tibial traumatic fracture admitted to our hospital from February 2019 to February 2020 were retrospectively analyzed.All patients were treated with internal fixation.Following up for 6 months after surgery,the incidence of traumatic arthritis was recorded,and the factors affecting the postoperative traumatic arthritis in patients with tibial traumatic fracture were analyzed.Results Among 52 patients with tibial traumatic fracture,20 cases(38.46%)had postoperative traumatic arthritis.Univariate and multivariate analysis showed that high energy injury,typeⅣtoⅥfracture,osteoporosis and non-compliance of postoperative exercise were the influencing factors of postoperative traumatic arthritis in patients with tibial traumatic fracture(OR>1,P<0.05).Conclusions High energy injury,typeⅣtoⅥfracture,osteoporosis and non-compliance of postoperative exercise can lead to postoperative traumatic arthritis in patients with tibial traumatic fracture.Targeted preventive measures should be taken to reduce the incidence of traumatic arthritis.
