生物型加长柄人工股骨头置换术对高龄不稳定性股骨转子间骨折患者患肢活动功能的影响 被引量:1

摘要 目的探讨生物型加长柄人工股骨头置换术治疗高龄不稳定性股骨转子间骨折的效果。方法选择我院收治的60例高龄不稳定性股骨转子间骨折患者,分为骨水泥组(30例)与生物组(30例)。骨水泥组行骨水泥假体置换术,生物组行生物型加长柄人工股骨头置换术。对比两组的围术期指标、血栓弹力图(TEG)参数及术后并发症情况。结果两组的围术期指标比较无统计学差异(P>0.05)。术后24 h,生物组的R值与K值低于骨水泥组,α角与MA值高于骨水泥组(P<0.05)。两组的术后并发症发生率比较无统计学差异(P>0.05)。结论生物型加长柄人工股骨头置换术与骨水泥假体置换术治疗高龄不稳定性股骨转子间骨折的效果相当,但前者可降低术后高凝血风险,促进患肢早日活动。 Objective To explore the effect of biological lengthened stem artificial femoral head replacement in the treatment of elderly unstable intertrochanteric fracture.Methods 60 elderly patients with unstable intertrochanteric fracture were divided into bone cement group(n=30)and biological group(n=30).The bone cement group reveived cemented prosthesis replacement,while the biological group received biological lengthened stem artificial femoral head replacement.The perioperative indicators,thromboelastography(TEG)parameters and postoperative complications were compared between the two groups.Results No statistical difference was found in the perioperative indicators between the two groups(P>0.05).24 hours after operation,the R value and K value of biological group were lower than those of bone cement group,and theαangle and MA value were higher than those of bone cement group(P<0.05).No statistical difference was found in the incidence of postoperative complications between the two groups(P>0.05).Conclusions Biological lengthened stem artificial femoral head replacement has similar effect with bone cement prosthesis replacement for elderly patients with unstable intertrochanteric fracture,but can reduce the risk of postoperative high blood coagulation and promote the early activity of affected limbs.
