摘要
目的探讨多学科协作护理对脑卒中患者神经功能及其主要照顾者心理弹性的影响。方法选择2017年2月到2020年2月诊断的脑卒中患者223例作为研究对象,按随机数表法将其分为对照组(n=109)和研究组(n=114),对照组予以常规护理,研究组在对照组基础上加用多学科协作护理模式。对比2组美国国立卫生研究院卒中量表(NIHSS)评分、日常生活活动(ADL)评分、Fugl-Meyer评估(FMA)评分、平均住院天数、平均住院费用、健康教育知识知晓率、并发症及心理弹性评分。结果治疗后,研究组患者满意度明显高于对照组(P<0.05);研究组NIHSS评分低于对照组,而ADL及FMA评分均高于对照组(P<0.05);研究组平均住院天数、平均住院费用明显低于对照组,而健康教育知识知晓率高于对照组(P<0.05),研究组的坚韧、自强、乐观维度得分均高于治疗前及对照组(P<0.05);研究组并发症发生率明显低于对照组并发症发生率(P<0.05)。结论通过多学科协作护理干预可促进脑卒中患者术后康复,改善患者神经功能,提高主要照顾者的心理弹性水平,对患者后期康复具有积极作用,值得在临床上应用及推广。
Objective To investigate the effects of multidisciplinary collaborative nursing on neurological function of patients with cerenral stroke and the psychological elasticity of primary caregivers.Methods A total of 223 patients with cerenral stroke who were treated in our hospital from February 2017 to February 2020 were enrolled in the study,who were divided into control group(n=109)and research group(n=114)according to random number table method.The patients in control group were treated bu routine nursing,however,the patients in research group,on the basis of control group,were treated by multidisciplinary collaborative nursing.The national institutes of health stroke scale(NIHSS)score,daily living activity(ADL)score,fugl-meyer assessment(FMA)score,average length of staying hospital,average cost of hospitalization,knowledge levels of health education,complications and mental resilience scores were observed and compared between the two groups.Results After treatment,the satisfaction degree of patients,knowledge levels of health education,ADL scores and FMA scores in resrach group were significantly higher than those in control group(P<0.05),however,the NIHSS scores average length of staying hospital and the average cost of hospitalization cost of stay were significantly lower than those in control group(P<0.05).Moreover the scores of optimistic emotions in research group were significantly higher than those before treatment and those in control group(P<0.05),but,the incidence rate of complications in research group was significantly lower than that in control group(P<0.05).Conclusion Multidisciplinary collaborative nursing intervention can promote the postoperative rehabilitation of patients with cerenral stroke,and improve the levels of psychological elasticity of primary caregivers,and play a positive role in the late recovery of patients,which is worthy of clinical application and promotion.
