摘要
目的探讨直肠癌(RC)患者术后直肠阴道瘘(RVF)发生状况及影响因素。方法采用前瞻性随机试验方法,选取50例RC患者作为研究对象,均行外科手术治疗。统计分析患者的术后RVF发生情况,对RC患者术后发生RVF的影响因素进行单因素及多因素Logistic回归分析。结果50例RC患者术后发生RVF 11例,占22.00%,将其纳入发生组;未发生RVF 39例,占78.00%,将其纳入未发生组。两组吻合方式、术前放射治疗、肿瘤距齿状线距离、绝经、联合脏器切除情况对比,差异有统计学意义(P<0.05);两组年龄、手术时间、合并盆腔感染情况对比,差异无统计学意义(P>0.05)。经Logistic回归分析,双吻合术、术前放射治疗、肿瘤距齿状线距离≤10 cm、绝经、联合脏器切除均为RC患者术后发生RVF的影响因素(OR>1,P<0.05)。结论RC患者术后RVF的发生率较高,受多种因素影响,临床需针对其相关因素给予干预。
Objective To discuss the occurrence and influencing factors of rectovaginal fistula(RVF)after surgery for patients with rectum cancer(RC)surgery.Methods A prospective randomized trial was conducted to select 50 patients with RC as the research subjects,and all patients received surgical treatment.The occurrence of postoperative RVF was statistically analyzed,and univariate analysis and Logistic regression analysis were used to identify the influencing factors of postoperative RVF in RC patients.Results Among 50 patients with RC,11 cases(22.00%)developed RVF,which were included in occurrence group;and 39 cases(78.00%)without RVF,which were included in the non-occurrence group.There was statistically significant difference in anastomosis mode,preoperative radiotherapy,tumor distance from dentate line,menopause and combined organ resection between the two groups(P<0.05).There was no statistically significant difference in age,operation time and combined pelvic infection between the two groups(P>0.05).Logistic regression analysis showed that double anastomosis,preoperative radiotherapy,tumor distance from dentate line≤10 cm,menopause and combined visceral resection were all the influencing factors for postoperative RVF in RC patients(OR>1,P<0.05).Conclusion The incidence of postoperative RVF in RC patients is high,which is affected by a variety of factors,and clinical intervention should be given according to the relevant factors.
