摘要
目的探讨地诺孕素对育龄期腹腔镜卵巢子宫内膜异位囊肿患者术后复发的抑制效果及卵巢储备功能的影响。方法选取86例在2019年1-8月于我院因“卵巢子宫内膜异位囊肿”住院行“腹腔镜卵巢囊肿剥离手术”治疗后的20~30岁育龄期女性。其中治疗组43例予以地诺孕素片2 mg/d口服,对照组43例予以皮下注射醋酸亮丙瑞林微球3.75 mg/4周,连续6个周期。观察治疗前及治疗停药3个月后妇科肿瘤指标CA125、CA199,卵巢储备功能AMH、AFC,B超复查卵巢囊肿复发情况,及月经停闭率、恢复率。结果治疗后停药3个月,治疗组妇科肿瘤指标CA125低于对照组(P<0.05)、CA199差异无统计学意义(P>0.05),治疗组卵巢储备功能AMH高于对照组(P<0.05)、AFC差异无统计学意义(P>0.05),B超复查卵巢囊肿,治疗组复发率与对照组相比,差异无统计学意义(P>0.05),治疗组月经停闭率显著低于对照组(16.30%vs.37.21%,P<0.05),但两组月经恢复率间的差异无统计学意义。结论地诺孕素对卵巢子宫内膜异位囊肿术后复发具有良好的抑制作用,停药后卵巢储备功能可迅速恢复,有利于育龄期女性后续备孕。
Objective To evaluate the effects of dienogest on postoperative recurrence and ovarian reserve function in patients with ovarian endometriosis cyst.Methods Eighty-six patients with ovarian endometriosis cyst were enrolled in this study and divided into two groups:dienogest group(n=43)and control group(n=43).All the patients underwent laparoscopic ovarian cyst dissection and were in childbearing age ranging from 20 to 30 years.The patients in dienogest group were given dienogest treatment with 2 mg per day.The patients in control group were injected by leuperirin acetate microspheres 3.75 mg every four weeks for six cycles.Observe the levels of CA125,CA199,AMH,AFC,postoperative recurrence,the rate of amenorrhea and menstrual recovery rate before and 3 months after treatment.Results In comparison with control group,the levels of CA125 and the rate of amenorrhea in dienogest group were decreased 3 months after treatment,while levels of AMH were increased.There was no significant difference in the levels of CA199,AFC,postoperative recurrence rate or the rate of menstrual recovery rate between these two groups(P>0.05).Conclusion Dienogest will be beneficial to the pregnancy preparation in childbearing patients with ovarian endometriosis cyst,which may be due to its ability to inhibit postoperative recurrence and improve ovarian reserve function.
