摘要
目的比较组合式输尿管软镜碎石术(FURL)和微通道经皮肾镜取石术(MPC-NL)在输尿管上段结石治疗中的疗效及安全性方法选取本院2017年1月至2017年12月收治的123例输尿管上段结石患者,将其随机分为两组,其中59例行FURL治疗(FURL组),64例行MPCNL治疗(MPCNL组)。比较两组患者的手术时间、术后住院时间、并发症发生率及结石残留率。结果FURL组的手术成功率为94.92%,MPCNL组的手术成功率为100%;两组手术时间比较,差异无统计学意义[(63.52±25.10)min vs.(6Z 47±15.86)min,P>0.05];两组结石清除率比较,差异无统计学意义[96.61%(57/59)vs.98.44%(63/64),P>0.05];FURL组的术后住院时间少于FUKL组,差异有统计学意义(P<0.05);FURI.组和MPCN1.组的并发症发生率分别为8.93%(5/56)和25.00%(16/64),差异有统计学意义(P<0.05)。结论MPC-NL和FUKL在输尿管上段结石患者的治疗中均安全有效,但FURL较MPCNL创伤小、术后并发症发生率低、住院时间更短。
Objective To compare the efficacy and safety of combined flexible ureteroscope lithotripsy(FURL)and micro-channel percutaneous nephrolithotomy(MPCNL)in the treatment of upper ureteral calculi.Methods From January 2017 to December 2017,123 patients with upper ureteral calculi were divided into two groups,59 patients undenvent FURL,64 patients underwent MPCNL.The operation time,postoperative complications,postoperative hospitalization time,postoperative residual rate of the two groups were compared.Results The surgical success rate was 94.92%in the FURL group and 100%in the MPCNL group.The operation time was(63.52±25.10)min and(62.47±15.86)min,respectively,there was no significant difference(P>0.05).The stone clearance rates were 96.61%(57/59)and 98.44%(63/64),respectively,with no significant difference(P>0.05).The incidence of complications was 8.93%(5/56)and 25.00%(16/64),respectively,the difference was statistically significant(P<0.05).The length of postoperative hospital stay in FURL group was less than that in MPCNL group,the difference was statistically significant(P<0.05).Conclusions Both MPCNL and F URL are safe and effective in the treatment of patients with upper ureteral calculi,hut FURL has less trauma,lower incidence of postoperative complications and shorter hospital stay than MPCNL.
