摘要
目的观察柴胡乌梅丸治疗非咳嗽变异性哮喘激素敏感性咳嗽寒热错杂证的临床效果。方法选取2018年9月至2020年6月于广州医科大学附属中医医院丘梅清教授门诊就诊的慢性咳嗽患者,经病史、症状、过敏原筛查及诊断性治疗诊断激素敏感性咳嗽;经支气管激发试验排除咳嗽变异性哮喘;经中医辨证“寒热错杂证”患者56例,男30例,女26例,年龄(46.2±11.6)岁。采用随机数字表法将其分为治疗组(26例)和对照组(30例)。对照组吸入糖皮质激素,治疗组口服柴胡乌梅丸汤剂,疗程4周,停药后随访1个月。观察两组治疗有效率、咳嗽症状积分、停药1个月后复发率以及不良反应发生率。结果治疗组总有效率优于对照组[88.46%(23/26)比73.33%(22/30),Z=-2.00,P=0.045]。治疗后,治疗组和对照组日间、夜间咳嗽症状积分皆较治疗前均明显改善[治疗组:日间(2.19±0.85)分比(0.92±0.84)、夜间(2.35±0.63)分比(0.88±0.77)分;对照组:日间(2.17±0.87)分比(1.53±0.90)分、夜间(2.33±0.71)分比(1.57±0.90)分;均P<0.05],且治疗组优于对照组(均P<0.05);治疗停止后随访1个月,治疗组和对照组咳嗽复发率差异无统计学意义[26.92%(7/26)比30.00%(9/30),χ^(2)=0.065,P=0.799]。结论柴胡乌梅丸治疗非咳嗽变异性哮喘激素敏感性咳嗽寒热错杂证疗效肯定,能够尽快缓解患者咳嗽症状,同时安全可靠,无明显不良反应。
Objective To observe the clinical effect of Chaihu Wumei pills for mixed cold and heat syndrome of corticosteroid responsive cough caused by non-cough variant asthma.Methods Fifty-six chronic cough patients diagnosed with mixed cold and heat syndrome by syndrome differentiation and with corticosteroid responsive cough by disease history,symptoms,allergen test,and diagnostic treatment,who were treated at Qiu Meiqing's Clinic of Hospital of Traditional Chinese Medicine Affiliated to Guangzhou Medical University from September,2018 to June,2020,were selected,including 30 males and 26 females,and they were(46.2±11.6)years old.They were divided into a treatment group(26 cases)and a control group(30 cases)by the random number table method.The control group inhaled glucocorticoids,and the treatment group orally took Chaihu Wumei pills,for four weeks;and the patients were followed up for one month after drug discontinuation.The treatment efficacies,cough symptom scores,recurrence rates 1 month after drug discontinuation,and incidences of adverse reactions were compared between the two groups.Results The treatment efficacy in the treatment group was higher than that in the control group[88.46%(23/26)vs.73.33%(22/30),Z=-2.00,P=0.045].The day and night cough symptom scores were better after than before the treatment in the treatment group[(2.19±0.85)vs.(0.92±0.84)and(2.35±0.63)vs.(0.88±0.77);both P<0.05]and in the control group[(2.17±0.87)vs.(1.53±0.90)vs.(2.33±0.71)vs.(1.57±0.90);both P<0.05],and were better in the treatment group than in the control group after the treatment(both P<0.05).During the one month follow-up after the treatment,there was no statistical difference in cough recurrence rate between the treatment group and the control group[26.92%(7/26)vs.30.00%(9/30),χ^(2)=0.065,P=0.799].Conclusion Chaihu Wumei pills for mixed cold and heat syndrome of corticosteroids responsive cough caused by non-cough variant asthma is effective and safe and can relieve the patients'cough,with no obvious adverse reacti
