摘要
【目的】探讨提拉钩技术侧卧位下股骨近端防旋髓内钉(proximal femoral nail antirotation,PFNA)治疗老年股骨粗隆间骨折的疗效。【方法】选择2017年3月至2020年5月提拉钩技术侧卧位下采用PFNA治疗的44例老年股骨粗隆间骨折患者。记录患者的手术时间、术中总出血量、骨折愈合时间、术后并发症发生等情况并分析。【结果】44例患者手术时间(48±13)min,术中出血量(67±23)ml。患者随访时间8~24个月;骨折愈合时间8~14周;术后8~15周可完全负重行走。根据Harris评分标准评定患者的髋关节功能,优良率为95.5%。【结论】提拉钩技术侧卧位下PFNA是治疗老年股骨粗隆间骨折的有效方法,具有手术时间短、复位满意、出血量少等优势。
【Objective】To investigate the clinical effect of proximal femoral nail antirotation(PFNA)in the lateral position using the pulling hook technique for treatment of intertrochanteric fracture of the femur in the elderly.【Methods】Between March 2017 to May 2020,44 patients with intertrochanteric fractures were treated with PFNA in the lateral position using the pulling hook technique.The operation time,total bleeding,fracture healing time,complications and others were recorded and analyzed.【Results】The operation time was(48±13)min,and the bleeding volume was(67±23)ml.All patients were followed-up for 8~24 months;fracture healing time was 8~14 weeks and the time to full weight-bearing was 8~15 weeks postoperatively.According to the Harris score of the hip joint,the excellent rate was 95.5%.【Conclusion】PFNA using the pulling hook technique is an effective method for the treatment of intertrochanteric fractures in the elderly.It has the advantages of short operation time,good reduction,and less bleeding when using the pulling hook technique.It is particularly suitable for grassroots hospitals without traction bed equipment.
