超声检测下腔静脉参数在椎管内麻醉围术期容量状态评估中的应用价值 被引量:1

摘要 目的探讨超声检测下腔静脉参数在椎管内麻醉围术期容量评估中的应用价值。方法选取2019年1月至2020年3月100例椎管内麻醉手术患者为研究对象,采用简单随机化法将其分为对照组(n=50)和观察组(n=50)。对照组实施常规平均动脉压(MAP)、中心静脉压(CVP)等检测来评估椎管内麻醉围术期容量状态,观察组在对照组基础上实施超声检查来评估椎管内麻醉围术期容量状态。比较两组患者的低血压发生率、去氧肾上腺素使用率、寒战发生率、液体补入量、液体补入时间及补液前、后的MAP、CVP、下腔静脉扩张指数(dIVC)。结果观察组的低血压发生率、去氧肾上腺素使用率、寒战发生率均低于对照组,差异具有统计学意义(P<0.05)。观察组的液体补入量少于对照组,液体补入时间短于对照组,差异具有统计学意义(P<0.05)。补液后,两组的MAP、CVP均较补液前升高,dIVC均较补液前降低,且观察组的MAP、CVP高于对照组,dIVC低于对照组,差异具有统计学意义(P<0.05)。结论超声检测下腔静脉参数,可较好地反映椎管内麻醉患者围术期的容量状态,指导其接受相应的容量治疗干预,以此降低低血压、寒战等的发生率,具有一定的应用和推广价值。 Objective To explore the application value of ultrasonic detection of inferior vena cava parameters in the evaluation of perioperative volume state of intraspinal anesthesia.Methods A total of 100 patients undergoing intraspinal anesthesia operation from January 2019 to March 2020 were selected as the research objects.The patients were divided into control group(n=50)and observation group(n=50)by simple randomization.The control group performed routine mean arterial pressure(MAP)and central venous pressure(CVP)detection to evaluate the perioperative volume state of intraspinal anesthesia,and the observation group performed ultrasound detection on the basis of the control group to evaluate the perioperative volume state of intraspinal anesthesia.The incidence of hypotension,the use rate of deoxyepinephrine,the incidence of shivering,the amount of liquid supplement,the time of liquid supplement,MAP,CVP and dilation inferior vena cava(dIVC)before and after fluid supplement were compared between the two groups.Results The incidence of hypotension,the use rate of deoxyepinephrine and the incidence of shivering in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The amount of liquid supplement in the observation group was less than that in the control group,the time of liquid supplement was shorter than that in the control group,and the differences were statistically significant(P<0.05).After liquid supplement,the MAP and CVP of the two groups were higher than those before liquid supplement,the dIVC were lower than that before liquid supplement,the MAP and CVP of the observation group were higher than those of the control group,the dIVC was lower than that of the control group,and the differences were statistically significant(P<0.05).Conclusion Ultrasonic detection of inferior vena cava parameters can better reflect the perioperative volume state of patients under intraspinal anesthesia,guide them to receive corresponding volume treatment inter
