摘要
目的比较三种不同术式矫正白内障合并角膜散光的临床疗效。方法将2020年1月至2020年12月就诊于我院的老年性白内障合并散光(散光≥1.50 D)患者分为3组,均行白内障超声乳化术,其中联合Toric人工晶体植入术Toric组25例(25眼),透明角膜松解切开术CRIS组25例(25眼)和行陡峭轴透明角膜主切口组25例(25眼)。结果 (1)三组总散光量在术后均显著减少(P<0.01)。(2)Toric组角膜散光在术后与术前相比差异无统计学意义(P>0.05)。CRIS组和陡峭轴切口组术后3个月角膜散光与术前相比均有明显差异(P<0.01)。Toric人工晶体术后1月与术后3月之间的晶体旋转度无显著(P>0.05)。(3)三组术后3个月UCVA、BCVA较术前比均显著提高(P<0.05),三组术后3个月的BCVA无明显差异(P>0.05)。结论三种术式均是老年性白内障合并角膜散光的安全有效方法,且超声乳化摘除联合Toric IOL植入术的疗效最优。
Objective To compare the clinical efficacy of three different surgical methods in correcting cataract combined with corneal astigmatism. Methods Patients with senile cataract complicated with astigmatism(astigmatism ≥1.50 D) admitted to our hospital from January 2020 to December 2020 were divided into 3 groups, which underwent phacoemulsification, including Toric intraocular lens implantation(Toric group) 25 cases(25 eyes), clear corneal lysis(CRIS group) 25 cases(25 eyes), and steep-axis clear corneal incision(25 eyes) group 25 cases(25 eyes). Results(1)The amount of astigmatism in three groups decreased significantly after operation(P<0.01).(2) There was no significant difference in corneal astigmatism between the Toric group after surgery and before surgery(P> 0.05).There were significant differences in the corneal astigmatism 3 months after operation between the CRIS group and the steep axis incision group(P<0.01). There was no significant difference in the rotation between 1 month and 3 months after surgery(P>0.05).(3) UCVA and BCVA at 3 months postoperatively were significantly increased in the three groups(P<0.05), but BCVA at 3 months postoperatively was not significantly different in the three groups(P>0.05). Conclusion All the three methods are safe and effective for senile cataract with corneal astigmatism, and phacoemulsification combined with Toric IOL implantation has the best effect.
