摘要
目的探讨动态三维超声造影(three-dimensional contrast-enhanced ultrasound,3D-CEUS)技术对肝局灶性病变(focal liver lesions,FLLs)动脉期增强表现的显示能力。方法根据入选及排除标准,最终210例经病理确诊或临床随访证实的FLLs患者,共210个病灶纳入研究。对同一肝内病灶依次行常规二维超声造影(twodimensional contrast-enhanced ultrasound,2D-CEUS)和动态3D-CEUS检查,观察两种造影模式下所得声像图,并对两种成像技术显示的动脉期病灶周围滋养血管、病灶增强模式和病灶内部血管形态进行比较。结果动态3DCEUS显示病灶增强模式与2D-CEUS无明显差异(P=0.887)。和2D-CEUS相比,动态3D-CEUS可观察到病灶周围更多的滋养血管,显示血管连续性更好、血管形态更清晰(P均<0.001)。动态3D-CEUS显示瘤内血管形态也优于常规2D-CEUS(P<0.001)。结论动态3D-CEUS可用于观察FLLs的动脉期增强模式。与2D-CEUS相比,动态3D-CEUS能更好地显示FLLs的血管特征。
Objective To investigate the enhancement features of dynamic three-dimensional contrastenhanced ultrasound(3D-CEUS)in focal liver lesions(FLLs).Methods According to the inclusion and exclusion criteria,210 patients with 210 FLLss confirmed by pathological examination or clinical followup were enrolled in our study.Both conventional two-dimensional contrast-enhanced ultrasound(2DCEUS)and dynamic 3D-CEUS were performed in the same lesion and the images acquired were assessed.The differences of the feeding vessels,enhancement pattern and intratumoral vessels of the same lesion on the two modes were compared statistically.Results Dynamic 3D-CEUS was similar to 2D-CEUS(P=0.887)with respect to enhancement patterns.Compared with 2D-CEUS,dynamic 3D-CEUS provided more detailed information about the number,continuity and morphology of the feeding vessels(P<0.001).Dynamic 3D-CEUS was also superior to 2D-CEUS in the display of intratumoral vessels(P<0.001).Conclusion Dynamic 3D-CEUS is an useful tool for displaying the enhancement patterns in arterial phase of FLLss.Compared with 2D-CEUS,dynamic 3D-CEUS provides additional information of vascular characteristics in the diagnosis of FLLss.
