摘要
目的观察基于"脊-盆-膝"整体诊疗模式的推拿手法对膝骨关节炎患者关节僵硬程度的影响。方法将80例膝骨关节炎患者随机分为治疗组和对照组,每组40例。对照组采用传统局部推拿手法治疗,治疗组采用基于"脊-盆-膝"整体诊疗模式的推拿手法治疗;两组均连续治疗1个疗程,比较西安大略和麦克马斯特大学骨关节炎指数(WOMAC)(僵硬部分)评分、关节活动度、疼痛视觉模拟评分(VAS)的变化情况,以及腰椎、骨盆症状发生率。结果(1)治疗前后组内比较,两组WOMAC(僵硬部分)评分、关节活动度、VAS评分差异均有统计学意义(P<0.05);组间治疗后比较,治疗组WOMAC(僵硬部分)评分低于对照组(P<0.05)。(2)治疗后,两组合并腰椎、骨盆症状患者占比均较治疗前降低(P<0.05),且治疗组较对照组降低更为明显(P<0.05)。结论基于"脊-盆-膝"整体诊疗模式的推拿手法治疗膝骨关节炎疗效满意,可改善下肢力线,减轻膝关节僵硬及疼痛程度,增加关节屈伸范围。
Objective To investigate the effect of massage manipulation on joint stiffness of patients with knee osteoarthritis based on the"spine-pelvis-knee"holistic diagnosis and treatment pattern.Methods Eighty patients with knee osteoarthritis were randomly divided into treatment group(n=40)and control group(n=40).The control group was administered with local massage manipulation,while the treatment group was administered with massage manipulation based on the"spine-pelvis-knee"holistic diagnosis and treatment pattern.The two groups were treated continuously for one course of treatment,and the changes of the Western Ontario MacMaster(WOMAC)stiffness score,joint range of motion,visual analogue scale(VAS)pain score and the incidence of lumbar and pelvic symptoms were compared.Results(1)According to the intra-group comparison before and after treatment,there were significant differences in WOMAC stiffness scores,joint range of motion,and VAS scores in both groups(P<0.05);After treatment,the WOMAC stiffness score in the treatment group was lower than that in the control group(P<0.05).(2)After treatment,the proportion of patients with symptoms of lumbar vertebra or pelvis in the two groups decreased(P<0.05),and the proportion in the treatment group decreased more significantly than that in the control group(P<0.05).Conclusion Massage manipulation based on the"spine-pelvis-knee"holistic diagnosis and treatment pattern is effective in treating knee osteoarthritis as it can improve the lower limb alignment,reduce the stiffness and pain of knee joint,and increase the range of joint flexion and extension.
