摘要
目的探讨彩色多普勒超声在足背部穿支皮瓣术前定位中的临床应用价值,通过与术中观察到的血管走行及皮穿支的数据进行对比,从而为皮瓣的切取提供参考。方法2018年3月-2019年4月,对12例手部软组织缺损并伴有骨或肌腱外露的患者,采用足背部穿支皮瓣游离修复。术前应用彩色多普勒超声定位足背部动脉走行及皮支位置,术中观察定位的准确性。结果12例通过术前彩色多普勒超声定位足背动脉穿支准确,术中均被采用。术后仅1例皮缘部分坏死,其余皮瓣均成活良好。随访5~8个月,皮瓣质地及外形良好,无臃肿,其颜色与周围皮肤相似。结论术前彩色多普勒超声对血管及皮支定位在游离足背部动脉穿支皮瓣的切取中可提供技术支持,术中对皮支处的组织加以保护,其余部分可快速切取,为手术顺利进行提供了保障,值得临床推广应用。
Objective To study the clinical value of Color Doppler ultrasound in the preoperative localization of dorsalis pedis perforator flap,and to provide reference for the cutting of flaps by comparing the vascular route and cutaneous perforation data during the operation.Methods From March 2018 to April 2019,12 patients with bone and tendon exposure were treated by dorsalis pedis perforator flap.Color Doppler ultrasound was used to locate the route of dorsalis pedis artery and the location of cutaneous branches before operation.Results Twelve patients were used to locate the perforator of dorsalis pedis artery by preoperative Color Doppler ultrasound.There was only 1 case of marginal necrosis after operation,and all the other flaps survived well.The follow-up time was 5~8 months,and it was found that the skin flap had good texture and shape,without bloating,and its color was similar to the surrounding skin.The donor area was repaired with full thickness skin graft.Conclusion Preoperative Color Doppler ultrasound provided technical support for the dissection of vascular and cutaneous branches in dorsalis pedis perforator flap.During the operation,the tissues at the cutaneous branches were protected,and the remaining parts can be cut off quickly,which provided guarantee for the operation and was worthy of application in clinical practice.
