摘要
目的探讨子宫动脉栓塞治疗产后大出血的效果及安全性。方法选取本院2018年1月至2020年1月收治的产后大出血患者80例,随机分为对照组和观察组,各40例。对照组采用子宫动脉结扎术治疗,观察组采用子宫动脉栓塞术(明胶海绵颗粒栓塞)治疗,比较两组止血效果、临床指标、子宫切除及并发症发生情况。结果观察组止血总有效率为95.00%,高于对照组的72.50%,差异有统计学意义(P<0.05);观察组手术时间、住院时间、下床活动时间均短于对照组,术后24 h出血量、阴道出血量均少于对照组,差异有统计学意义(P<0.05);观察组子宫切除率低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率(12.50%)低于对照组(32.50%),差异有统计学意义(P<0.05)。结论采用子宫动脉栓塞术治疗产后大出血患者止血效果显著,且可有效降低子宫切除率,利于患者术后恢复,安全性较高,值得临床推广运用。
Objective To investigate effect and safety of uterine artery embolization in the treatment of postpartum hemorrhage.Methods 80 pa-tients with postpartum hemorrhage treated in our hospital from January 2018 to January 2020 were randomly divided into control group and observa-tion group,with 40 cases in each group.The control group was treated with uterine artery ligation and the observation group was treated with uterine artery embolization(gelatin sponge particle embolization).The hemostatic effect,clinical indexes,hysterectomy and complications were compared between the two groups.Results The total effective rate of hemostasis in the observation group was 95.00%,which was higher than 72.50%in the control group(P<0.05);the operation time,hospital stay and out of bed activity time in the observation group were shorter than those in the control group,the amount of bleeding and vaginal bleeding 24 hours after operation were less than those in the control group(P<0.05);the hysterectomy rate in the observation group was lower than that in the control group(P<0.05);the incidence of complications in the observation group(12.50%)was lower than that in the control group(32.50%),the difference was statistically significant(P<0.05).Conclusion Uterine artery embolization in patients with postpartum hemorrhage can effectively reduce the hysterectomy rate,facilitate postoperative recovery,and the safety is high.It is wor-thy of clinical application.
