摘要
目的探讨放散状冲击波对颞下颌关节紊乱患者颞下颌关节紊乱紊乱指数(CMI)及视觉模拟评分(VAS)的影响。方法选取2019年1月至2020年6月于本院接受治疗的60例颞下颌关节紊乱患者作为研究对象,随机分为两组,每组30例。对照组使用聚焦式冲击波辅助物理手法进行治疗,观察组使用放散状冲击波结合物理手法治疗,比较两组CMI评分、VAS评分、最大开口度、不良反应发生率。结果治疗4周后,观察组CMI评分、VAS评分均低于对照组,最大开口度大于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率(10.00%)略低于对照组(16.67%),但差异无统计学意义。结论放散状冲击波和聚焦式冲击波均能改善颞下颌关节紊乱患者CMI评分、VAS评分以及最大开口度,但放散状冲击波治疗效果更显著,值得临床推广运用。
Objective To investigate the effect of diffuse shock wave on temporomandibular joint disorder index(CMI)and visual analog scale(VAS)in patients with temporomandibular joint disorder.Methods 60 patients with temporomandibular joint disorders who were treated in our hospital from January 2019 to June 2020 were selected as the research subjects,and they were randomly divided into two groups,with 30 cases in each group.The control group was treated with focused shock wave-assisted physical manipulation,and the observation group was treated with diffuse shock wave combined with physical manipulation.The CMI score,VAS score,maximum aperture,and incidence of adverse reactions were compared between the two groups.Results After 4 weeks of treatment,the CMI score and VAS score of the observation group were lower than those of the control group,and the maximum opening was greater than that of the control group,the difference was statistically significant(P<0.05);the incidence of adverse reactions in the observation group during the treatment period(10.00%)was slightly lower than that in the control group(16.67%),but the difference was not statistically significant.Conclusion Both the diffuse shock wave and the focused shock wave can improve the CMI index,VAS score and the maximum opening degree of patients with temporomandibular joint disorders,but the effect of the diffuse shock wave is more obvious,and it is worthy of clinical application.
