血清Bcl-2、Cox-2水平与FP方案治疗食管鳞癌患者预后的相关性 被引量:2

Correlation between Serum Bcl-2,Cox-2 Levels and Prognosis of FP Regimen in the Treatment of Patients with Esophageal Squamous Cell Carcinoma

摘要 目的观察食管鳞癌(ESCC)患者血清B细胞淋巴瘤2(Bcl-2)、环氧合酶2(Cox-2)表达,分析血清Bcl-2、Cox-2表达与FP方案治疗ESCC患者预后的关系。方法前瞻纳入接受FP方案治疗的97例ESCC患者为研究对象,于治疗6个周期,参照相关标准评估患者预后并分组;设计基线资料调查表,详细询问患者并记录研究所需基线资料,分别于化疗前、化疗6个月周期测定患者的实验室相关指标水平,重点比较血清Bcl-2、Cox-2表达,分析血清Bcl-2、Cox-2表达与FP方案治疗ESCC患者预后的关系。结果97例ESCC患者化疗6个周期Bcl-2、Cox-2水平均较化疗前降低(P<0.05);97例ESCC患者32例预后不良,占32.99%;预后不良ESCC患者CEA、Bcl-2、Cox-2水平均高于预后良好患者(P<0.05);组间其他基线资料比较差异无统计学意义(P>0.05);回归分析结果显示,化疗前癌胚抗原(CEA)、Bcl-2、Cox-2表达与ESCC患者FP方案治疗预后有关,各指标的过表达可能是ESCC患者FP方案治疗预后不良的风险因子(OR>1,P<0.05);绘制ROC曲线发现,Bcl-2、Cox-2预测ESCC患者FP方案治疗预后不良的AUC分别为0.863、0.876,均有一定预测价值,且二者的cut-off值分别取16.345 ng/l、74.895 mg/l时,预测价值最佳。结论ESCC患者经FP方案治疗后仍有一定预后风险,可能与化疗前Bcl-2、Cox-2过表达有关,考虑未来可通过检测患者化疗前Bcl-2、Cox-2水平,辅助评估并预测患者预后。 Objective To observe the expression of serum B-cell lymphoma 2(Bcl-2)and cyclooxygenase-2(Cox-2)in patients with esophageal squamous cell carcinoma(ESCC),to analyze the relationship between the expression of serum Bcl-2 and Cox-2 and the prognosis of FP regimen in the treatment of ESCC patients.Methods 97 ESCC patients who received FP regimen for treatment were prospectively included as the research subjects,after 6 cycles of treatment,the prognosis of the patients was evaluated according to the relevant criteria and divided them into groups;designed baseline questionnaire,the patients were asked in detail and the baseline data required for the study were recorded;the levels of laboratory related indexes were measured before and after 6 months of chemotherapy,the expression of serum Bcl-2 and Cox-2 was compared,the relationship between the expression of serum Bcl-2 and Cox-2 and the prognosis of FP regimen in the treatment of ESCC patients was analyzed.Results Of 97 cases of patients with ESCC after 6 cycles of chemotherapy,the levels of Bcl-2 and Cox-2 were decreased compared with those before chemotherapy(P<0.05);among 97 ESCC patients,32 had poor prognosis,accounting for 32.99%;the levels of CEA,Bcl-2 and Cox-2 in ESCC patients with poor prognosis were higher than patients with good prognosis(P<0.05);compared other baseline data between groups,there was no statistical significant difference(P>0.05);the results of regression analysis showed that the expression of carcinoembryonic antigen(CEA),Bcl-2 and Cox-2 were related to the prognosis of FP regimen in the treatment of ESCC patients,the overexpression of each index might be a risk factor for poor prognosis of FP regimen in the treatment of ESCC patients(OR>1,P<0.05);ROC curve showed that the AUC of Bcl-2 and Cox-2 in predicting poor prognosis of FP regimen in the treatment of ESCC patients were 0.863 and 0.876 respectively,which had certain predictive value,when the cut-off values of the 2 methods were 16.345 ng/L and 74.895 mg/L respectively,the prediction v
