摘要
目的:比较米库氯铵和苯磺顺阿曲库铵用于无痛纤维支气管镜检查术的效果。方法:拟行纤维支气管镜检查术患者100例,性别不限,年龄18~64岁,ASA分级Ⅰ或Ⅱ级,采用随机数字表法分为2组(n=50):米库氯铵组(M组)和苯磺顺阿曲库铵组(C组)。M组静脉注射米库氯铵0.15 mg/kg,C组静脉注射苯磺顺阿曲库铵0.1 mg/kg。记录肌松药起效时间、临床作用时间、恢复指数、恢复自主呼吸时间、拔除喉罩时间和出PACU时间。记录术中及术后并发症的发生情况。于入室静卧10 min(T_(1))、意识消失(T_(2))、喉罩置入(T_(3))、手术结束(T_(4))、拔除喉罩(T_(5))和出室(T_(6))时记录MAP、HR和SpO_(2)。结果:与C组比较,M组患者临床作用时间、恢复指数、恢复自主呼吸时间、拔除喉罩时间和出PACU时间缩短,总不良反应发生率降低(P<0.05),起效时间差异无统计学意义(P>0.05)。2组患者各时点MAP、HR和SpO_(2)比较差异无统计学意义(P>0.05)。结论:米库氯铵用于无痛纤维支气管镜检查术的效果优于苯磺顺阿曲库铵。
Objective To compare the efficacy of mivacurium versus cisatracurium in patients undergoing painless fiberoptic bronchoscopy.Methods A total of 100 patients of both sexes,aged 18-64 yr,of American Society of Anesthesiology physical status I orⅡ,scheduled for elective fiberoptic bronchoscopy were divided into 2 groups(n=50 each)using a random number table method:mivacurium group(M group)and cisatracurium group(C group).Mivacurium 0.15 mg/kg was injected intravenously in group M,and cisatracurium 0.1 mg/kg was injected intravenously in group C.The onset time of neuromuscular block(ThD95),the duration of neuromuscular block(TOFR25),recovery index(RI),recovery time of autonomous respiration,extubation time and time of discharge from postanesthesia care unit(PACU)were recorded.The occurrence of intraoperative and postoperative adverse reactions and complications were recorded.The mean arterial pressure(MAP),heart rate(HR)and SpO_(2) at restlessness at 10 min after entering the operating room(T_(1)),at loss of consciousness(T_(2)),when laryngeal mask airway was inserted(T_(3)),at the end of surgery(T_(4)),when laryngel mask airway was removed(T_(5)),and when the patients left the operating room(T_(6)).Results Compared with group C,TOFR25,RI,recovery time of autonomous respiration,extubation time and time of discharge from PACU were significantly shortened,the total incidence of adverse reactions was decreased(P<0.05),and no significant change was found in ThD95 in group M(P>0.05).There was no significant difference in MAP,HR and SpO_(2) at each time point between the 2 groups(P>0.05).Conclusion Mivacurium provides better efficacy than cisatracurium when used for painless fiberoptic bronchoscopy.
