摘要
目的探讨磁共振T_(2)^(*)mapping的全域直方图对评估宫颈癌脉管浸润情况的应用价值。方法回顾性分析经病理诊断为宫颈癌并进行T_(2)^(*)mapping检查的患者59例。将患者病灶按照脉管浸润(LVSI)状态分组,采用MaZda软件勾画兴趣区并进行直方图分析,对比两组T_(2)^(*)图像的灰度直方图参数,包括平均值(Mean)、方差(Variance)、偏度(Skewness)、峰值(Kurtosis)、第1百分位数(1th)、第10百分位数(10th)、第50百分位数(50th)、第90百分位数(90th)、第99百分位数(99th),绘制ROC曲线分析以上参数评估宫颈癌LVSI情况的诊断效能。结果宫颈癌LVSI阳性组的T_(2)^(*)mapping直方图参数Mean、Variance、10th、90th和99th值均明显高于阴性组(P<0.05),两组患者间的1th、Skewness和Kurtosis值无明显统计学差异。其中99th值的曲线下面积最大(0.924),其敏感性和特异性分别为90.5%、84.2%。结论基于T_(2)^(*)mapping的全域直方图分析有潜力应用于宫颈癌LVSI状态的评价。
Objective To investigate the value of T_(2)^(*)mapping histogram in the evaluation of lymphovascular space invasion(LVSI)in cervical cancer.Methods A total of 59 patients with cervical cancer diagnosed pathologically and underwent T_(2)^(*)mapping were retrospectively analyzed.The lesions were divided into groups according to the status of LVSI.The ROIs were drawn by Mazda software and histogram analysis was carried out.The gray histogram parameters of two groups of T_(2)^(*)mapping are compared,including Mean,Variance,Skewness,Kurtosis,1 percentiles(1 th),10 percentiles(10 th),50 percentiles(50 th),90 percentiles(90 th)and 99 percentiles(99 th),draw ROC curve and analyze the above parameters to evaluate the differential diagnosis efficacy of the status of LVSI.Results The Mean,Variance,10 th,90 th and 99 th values of LVSI positive group patients were significantly higher than those of the negative group(P<0.05).The 1 st,Skewness and Kurtosis values were not significantly different between the two group patients.Among them,the area under the curve of 99 th value is the largest(0.924),and its sensitivity and specificity are 90.5%and 84.2%,respectively.Conclusion The whole domain histogram based on T_(2)^(*)mapping has potential in the evaluation of the LVSI status of cervical cancer.
