摘要
目的:讨论纤维胆道镜联合术中B超在肝胆管结石手术中的临床应用价值和效果。方法:将我院于2019年3月-2020年3月收治进行肝胆管结石手术的患者120例纳入研究,并根据不同手术方式分为对照组和试验组两组,各60例。对照组采用常规纤维胆道镜取石手术;试验组在常规手术过程中应用纤维胆道镜的同时联合应用术中B超。比较两组治疗效果及残石复发率。结果:试验组患者仅出现无效1例,治疗总有效率为98.33%,对照组出现无效7例,治疗总有效率为88.33%,试验组优势更加明显,差异有统计学意义(P<0.05);试验组患者术后1年随访仅出现残石复发1例,复发率仅为1.67%;对照组出现残石复发8例,复发率为13.33%,差异有统计学意义(P<0.05)。结论:纤维胆道镜联合术中B超在肝胆管结石手术中具有较好的治疗效果,且可有效降低术后残石复发率,值得应用和推广。
Objective:to discuss the clinical value and effect of choledochofiberscope combined with intraoperative B ultrasound in the operation of hepatolithiasis.Methods:120 patients with hepatolithiasis in our hospital from March 2019 to March 2020 were included in the study,and they were divided into control group and experimental group according to different operation methods,60 cases in each group.The control group was treated with conventional choledochofiberscope;In the experimental group,the application of choledochofiberscope combined with intraoperative B ultrasound was used in the routine operation.The treatment effect and recurrence rate of residual stone were compared between the two groups.Results:The total effective rate was 98.33%in the experimental group and 88.33%in the control group.The advantage of the experimental group was more obvious,and the difference was statistically significant(P<0.05);In the experimental group,only 1 case of residual stone recurred after 1 year of follow-up,and the recurrence rate was only 1.67%;There were 8 cases of residual stone recurrence in the control group,the recurrence rate was 13.33%,the difference was statistically significant(P<0.05).Conclusion:Choledochofiberscope combined with intraoperative B-ultrasound in the treatment of hepatolithiasis has a good effect,and can effectively reduce the recurrence rate of postoperative residual stones,which is worthy of application and promotion.
