磁敏感成像在帕金森病和血管性帕金森综合征患者中的应用研究 被引量:2

Application of Susceptibility Imaging in Patients with Parkinson's Disease and Vascular Parkinson's Syndrome

摘要 目的探讨磁敏感加权成像(susceptibility weighted imaging,SWI)在血管性帕金森综合征(vascular parkinsonism,VP)和帕金森病(Parkinson's disease,PD)患者大脑内锥体外系各个核团的相位值及脑内微出血灶在临床应用中的鉴别诊断价值。方法方便收集于2018年6月—2020年6月期间在莆田学院附属医院门诊和住院的帕金森病患者(PD组,n=30例)、血管性帕金森综合征患者(VP组,n=28例),同时收集同期来该院体检的健康同龄老人(NC组,n=28名)作为对照组为研究对象,均行MRI的T2WI和SWI系列检查,比较3组患者壳核(PUT)、黑质致密带(SNc)、黑质网状带(SNr)、苍白球(Gp)、尾状核(CN)和红核(RN)的相位值,并记录脑微出血点(cerebral microbleeds,CMBs)的数量,采用单因素方差分析进行比较。结果PD组患者PUT、SNc、Gp相位值分别为(-0.409±0.008),(0.038±0.004),(-0.247±0.006)较VP组(-0.184±0.010),(0.060±0.034),(-0.130±0.004),对照组(-0.178±0.009),(0.062±0.004),(-0.126±0.008),有明显降低,差异有统计学意义(P<0.01),而VP组PUT、SNc、Gp相位值与对照组比较差异无统计学意义(P>0.05);对PD患者及VP组患者进行Hoehn-Yahr分级,各级PD患者PUT、SNc、Gp相位值之间差异有统计学意义(P<0.01);而IV级VP患者才出现SNc、SNr、Gp相位值降低,差异有统计学意义(P<0.01);CN、RN相位值在各级PD、VP患者之间均差异无统计学意义(P>0.05);VP组CMBs数目(7.11±2.657)个比PD组(0.53±0.973)个、对照组(0.25±0.585)个比较明显增多,差异有统计学意义(P<0.01),但PD组与对照组CMBs数目比较,差异无统计学意义(P>0.05)。结论应用SWI测量锥体外系各个核团相位值和记录CMBs个数对PD和VP的临床鉴别诊断提供一定的参考价值。 Objective To investigate the effects of susceptibility weighted imaging(SWI)in the extrapyramidal nuclei in the brain of patients with vascular parkinsonism(Vascular parkinsonism,VP)and Parkinson's disease(Parkinson's disease,PD).The differential diagnosis value of phase value and intracerebral microhemorrhage in clinical application.Methods Conveniently collected patients with Parkinson's disease(PD group,n=30 cases)and vascular Parkinson's syndrome patients(VP group,n=28 people)in the affiliated hopsital of Putian University from June 2018 to June 2020.At the same time,healthy elderly people of the same age(NC group,n=28 cases)who came to the hospital for physical examination during the same period were collected as the control group.They all underwent MRI T2WI and SWI serial examinations.Compared the phase values of putamen(PUT),substantia nigra compact zone(SNc),substantia nigra reticulum(SNr),globus pallidus(Gp),caudate nucleus(CN)and red nucleus(RN)in the three groups of patients,and the number of cerebral microbleeds(CMBs)was recorded and compared using one-way analysis of variance.Results The PUT,SNc,and Gp phase values of the PD group were respectively(-0.409±0.008),(0.038±0.004),(-0.247±0.006)compared with the VP group(-0.184±0.010),(0.060±0.034),(-0.130±0.004)and the control group(-0.178±0.009),(0.062±0.004),(-0.126±0.008)were significantly reduced,and the difference was statistically significant(P<0.01),while the PUT,SNc,and Gp phase values of the VP group were not statistically significantly different from those of the control group(P>0.05);Hoehn-Yahr classification for PD patients and VP patients,the differences in the PUT,SNc,and Gp phase values of PD patients at all levels were statistically significant(P<0.01);SNc,SNr,and Gp were only present in patients with grade IV of VP group decreased,and the difference was statistically significant(P<0.01);the phase values of CN and RN were not statistically significant differences among all levels of PD and VP patients(P>0.05);the number of CMBs i
