摘要
目的应用疾病诊断相关分组(DRG)对"脊柱手术"亚专科进行住院医疗服务评价,分析研究某医院近5年亚专科发展情况,为医院重点专科建设提供指导建议。方法以CN-DRG为分组方式,应用病例组合指数(CMI)、总权重、时间消耗指数、费用消耗指数、低风险和中低风险组死亡率对该医院"脊柱手术"亚专科进行分析。结果研究医院2019年亚专科病例数占市属三级医院26.12%,CMI高于市属三级医院;IB13、IB15总权重逐年增加,而IB23、IB25总权重逐年减低;时间消耗指数0.85,费用消耗指数高于市属三级医院4%;亚专科4个DRG死亡率均为0,低于市属三级医院平均水平。结论研究医院"脊柱手术"亚专科的诊疗能力处于全市较高水平,收治病例难度高于市属三级医院平均水平,但部分病组总权重下降,应注意保持其医疗服务的能力;时间消耗指数较低,费用消耗指数偏高,亚专科医疗服务安全性高。
Objective Evaluating the inpatient medical service of"spinal surgery"sub specialty by using the diagnosis-related groups,analyzing the development of the subspecialty in a hospital in recent 5 years,and providing recommendations for the development of the clinical key specialty.Methods Analyzing patients discharged from"spinal surgery"subspecialty using case-mix index(CMI),total weight,time-consuming index,cost-consuming index,mortality rate of low-risk group,low and medium risk mortality according to CN-DRG packet mode.Results In2019,the number of subspecialty cases in the research hospital accounted for 26.12%of municipal tertiary hospitals,CMI was higher than that of municipal tertiary hospitals;the total weight of IB13 and IB15 increased year by year,while the total weight of IB23 and IB25 decreased year by year;the time consumption index was 0.85,and the cost consumption index was 4%higher than that of municipal tertiary hospitals;the death rate of four subspecialty DRG was 0,lower than the average level of municipal tertiary hospitals.Conclusion The diagnosis and treatment ability of the"spinal surgery"sub specialty of the research hospital is at a higher level in the whole city,and the difficulty of accepting cases is higher than the average level of municipal tertiary hospitals,but the total weight of some disease groups is decreased.Attention should be paid to maintaining the ability of medical service;the time consumption index was low,the cost consumption index was high,and the safety of the medical service of the subspecialty was high.
