摘要
目的:分析2017-2020年本院368例药物不良反应(ADR)发生的特点、规律和相关因素,提高ADR的监测和临床合理用药水平。方法:对医院上报国家药物不良反应监测中心的368例不良反应进行回顾性分类统计分析。结果:ADR中大多为一般不良反应(75.54%),60岁以上的人群所占比例最大(228例,61.96%),由静脉给药引起的ADR最多(257例,69.84%),发生ADR的药品主要为抗感染药物(45.11%),其次为心血管系统药物(22.01%)和神经系统药物(11.14%)。抗感染药物中主要为抗菌药物,帕累托图显示抗菌药物ADR主要因素(A类)药品为喹诺酮类和头孢类。ADR主要累及皮肤及附件,其次为消化系统。结论:应加强对老年患者和静脉给药途径药物不良反应的监测,并将抗菌药物、心血管药物和神经系统用药作为ADR重点监测药物,以减少ADR发生,保障患者用药安全。
Objective:To analyze the characteristics,laws and the factors of 368 cases of adverse drug reaction (ADR) in our hospital from 2017 to 2020,so as to provide reference for improving the ability of monitoring and safe application of drugs in clinic.Method:A total of 368 cases of ADR reported to the national ADR monitoring center were analyzed retrospectively.Result:The constituent ratio of general ADR (75.54%) was the highest,the constituent ratio of the ADR occurred in patients over 60 years old (228 cases,61.96%) was the highest,intravenous administration (257 cases,68.75%) was the most in respect of administration routes.Anti-infection agents (45.11%) occupied the highest proportion in categories of drugs,followed by cardiovascular drugs (22.01%) and nervous system drugs (11.14%).Antibacterials drugs were the major anti-infection agents causing ADR.Pareto analysis showed that the major drug factors involved quinolones and cephalosporins.Major organs/systems involved in ADR were lesion of skin and its appendents,followed by digestive system.Conclusion:In order to reduction of ADR occurrence and ensure the safety of clinic therapy,clinicians and clinic pharmacists should enhance the monitor of ADR of antibacterials,cardiovascular drugs and nervous system drugs.
