摘要
目的观察综合护理干预用于预防腔内心电图经外周静脉穿刺中心静脉(PICC)尖端定位并发症的应用效果。方法选取鄂东医疗集团黄石市中心医院2018年3月至2019年9月接收的恶性肿瘤患者162例,按照临床护理方法的差异分为综合组与常规组,每组81例。两组患者均接受相同的腔内心电图PICC尖端定位治疗,此外,常规组采取常规的腔内心电图PICC尖端定位护理干预法,综合组采取综合护理干预措施,观察两组PICC尖端到位率、置管成功后的并发症发生率以及出院前1 d的情绪状态与生活质量。结果综合组患者PICC尖端定位的到位率98.77%明显优于常规组的87.65%,而综合组置管成功后的并发症总发生率(39.51%)明显低于常规组的77.78%,同时,综合组出院前1 d的焦虑自评量表(SAS)得分与抑郁自评量表(SDS)得分明显低于常规组,生活质量评分与护理总满意率明显高于常规组,差异有统计学意义(均P<0.05)。结论综合护理干预不仅对腔内心电图PICC尖端定位并发症具有良好的预防效果,还能显著提高PICC尖端的到位率和护理满意率,改善患者的不良情绪与生活质量。
Objective To observe the effect of comprehensive nursing intervention in preventing complications of intracavitary electrocardiogram(ECG)through peripheral inserted central vein(PICC)tip localization.Methods A total of 162 cases of malignant tumor patients received from this hospital from March 2018 to September 2019 were selected.According to the differences in clinical nursing methods,they were divided into comprehensive group and routine group,with 81 cases in each group.Patients in both groups received the same intracaval ECG PICC tip positioning treatment,in addition,the conventional group was treated with conventional intracaval ECG PICC tip positioning nursing intervention,and the comprehensive group was treated with comprehensive nursing intervention measures.PICC tip reach rate,complication rate after successful catheterization,emotional state and quality of life 1 day before discharge were observed in two groups.Results The reach rate of PICC tip localization in the comprehensive group(98.77%)was significantly better than that in the conventional group(87.65%),and the total incidence of complications after successful catheterization in the comprehensive group(39.51%)was significantly lower than that in the conventional group(77.78%).At the same time,the scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)in the comprehensive group were significantly lower than those in the routine group,and the scores of quality of life and total satisfaction rate of nursing were significantly higher than those in the routine group,the differences were statistically significant(P<0.05).Conclusions Comprehensive nursing intervention not only has a good preventive effect on complications of intramural ECG PICC tip positioning,but also can significantly improve the PICC tip arrival rate and nursing satisfaction rate,improve patients'bad mood and quality of life.
