急性淋巴细胞白血病患儿QKI和Oct4及Bcl-2 mRNA的表达意义研究 被引量:1

Research on the expression significance of QKI,Oct4 and Bcl-2 mRNA in children with acute lymphoblastic leukemia

摘要 目的探讨QKI、Oct4和B细胞淋巴瘤/白血病-2基因(Bcl-2)在急性淋巴细胞白血病(ALL)患儿中的表达及意义,为ALL防治及靶向治疗提供参考。方法采集2017-04-01-2020-02-15广州市妇女儿童医疗中心收治的初诊ALL患儿196例作为观察组,并按临床危险因素分级分为高危组(n=24)、中危组(n=43)和标危组(n=129),均接受化疗,并依据化疗反应情况分为反应好组(n=141)与反应差组(n=55);同期就诊非恶性血液病患儿50例作为对照组。qRT-PCR测定骨髓单个核细胞(BMMC)内QKI(QKI5、QKI6)、Oct4和Bcl-2mRNA表达,比较性别、危险分级和治疗反应与上述基因表达相关性,并于化疗后采骨髓液标本测定上述基因表达,比较治疗前后各基因表达的变化,Pearson相关分析法分析ALL患儿QKI、Qct4和Bcl-2mRNA表达相关性。结果观察组QKI5mRNA相对表达量为1.365±0.264,低于对照组的2.431±0.516(t=-20.364,P<0.001),Oct4和Bcl-2mRNA相对表达量分别为1.798±0.351和3.714±1.026,较对照组的0.675±0.212和0.361±0.105高(t值分别为21.620和23.043,均P<0.001);男性ALL患儿QKI5、QKI6、Oct4和Bcl-2mRNA相对表达量分别为1.373±0.334、1.865±0.587、1.809±0.401和3.733±1.146,与女性ALL患儿的1.358±0.275、1.882±0.614、1.757±0.441和3.698±1.212比较,差异无统计学意义,t值分别为0.325、-0.196、0.858和0.206,均P>0.05;高危组QKI5mRNA为0.789±0.154,低于中危组(1.321±0.403)与标危组(1.957±0.526),F=77.791,P<0.001;高危组Oct4和Bcl-2mRNA分别为2.987±0.452和5.576±1.265,高于中危组(1.866±0.447,3.651±0.714)与标危组(1.575±0.266,2.152±0.657),F值分别为172.230和228.172,均P<0.001;化疗前ALL患儿QKI5mRNA为1.365±0.264,低于化疗后的1.998±0.532(t=-14.922,P<0.001),化疗前Oct4和Bcl-2mRNA分别为1.798±0.351和3.714±1.026,高于化疗后的0.975±0.247和1.579±0.456,t值分别为26.845和26.622,均P<0.001;反应好组QKI5mRNA为1.965±0.417,高于反应差组的0.895±0.225(t=18.015,P<0.001),反应好组Oct4和Bc Objective To explore the expression and significance of QKI,Oct4,and B-cell lymphoma/leukemia-2 gene(Bcl-2)in children with acute lymphoblastic leukemia(ALL)to provide references for the prevention and targeted treatment of ALL.Methods Totally 196 newly diagnosed ALL children from April 1,2017 to February 15,2020 were collected as the observation group,and according to the clinical risk factors,they were divided into high-risk group(n=24),intermediate risk group(n=43)and standard risk group(n=129).All of them received chemotherapy,and were divided into good response group(n=141)and poor response group(n=55)according to the response to chemotherapy;50 children with non-malignant hematological diseases during the same period were treated as the control group.qRT-PCR was used to determine the expression of QKI(QKI5,QKI6)mRNA,and Oct4,Bcl-2 mRNA of bone marrow mononuclear cells(BMMC).The expression of the above genes in ALL children with different genders,different risk grades and different treatment responses was compared,and bone marrow fluid samples were collected after chemotherapy to determine the expression of the above genes was compared before and after treatment.Pearson correlation analysis method was used to analyze the correlation of QKI,Qct4 and Bcl-2 mRNA expression in ALL children.Results The relative expression of QKI5 mRNA in the observation group was 1.365±0.264,which was lower than that in the control group(2.431±0.516),t=-20.364,P<0.001.The relative expression of Oct4 mRNA and Bcl-2 mRNA were 1.798±0.351 and 3.714±1.026,which were higher than those in the control group(0.675±0.212,0.361±0.105),t values were 21.620,23.043,both P<0.001.The relative expressions of QKI5 mRNA,QKI6 mRNA,Oct4 mRNA,and Bcl-2 mRNA in male children with ALL were 1.373±0.334,1.865±0.587,1.809±0.401,3.733±1.146,and those in female children with ALL were 1.358±0.275,1.882±0.614,1.757±0.441,3.698±1.212,with no statistically significant difference(t values were 0.325,-0.196,0.858,0.206,all P>0.05).QKI5 mRNA in the h
