摘要
目的:探讨腹腔镜袖状胃切除术对肥胖合并2型糖尿病患者牙周状况的影响。方法:分析2019年6月至2020年4月接受腹腔镜袖状胃切除术的肥胖合并2型糖尿病患者的临床资料,选取其中的34例牙周炎患者,男11例,女23例。分别于术前及术后3、6、12个月记录患者身高、体重,检测血糖水平、探诊深度、出血指数、菌斑指数、临床附着丧失,以评估患者牙周状况。结果:术后患者体重呈进行性下降,空腹血糖与糖化血红蛋白于术后3个月有显著改善。牙周健康评估结果显示,术前、术后各随访时点患者的探诊深度、临床附着丧失差异无统计学意义,但术后3、6、12个月的菌斑指数均较术前显著下降,术后6、12个月的出血指数较术前显著下降。结论:腹腔镜袖状胃切除术可在一定程度上改善肥胖合并2型糖尿病患者的牙周炎症。
Objective:To investigate the effect of laparoscopic sleeve gastrectomy on the periodontal status of obese patients with type 2 diabetes mellitus(T2DM).Methods:Obese patients with T2DM who underwent laparoscopic sleeve gastrectomy from Jun.2019 to Apr.2020 were initially included.Among them,34 patients(11 males and 23 females)with periodontitis were selected.Before operation and 3,6,12 months after surgery,the patients’height and weight was recorded.The blood glucose level was measured.At the same time,the patients'probing depth,bleeding index,plaque index and clinical attachment loss were also recorded as indicators of the patients’periodontal condition.Results:The weight of patients progressively decreased after operation.The level of fasting blood glucose and glycosylated hemoglobin had been significantly improved since 3 months after the operation.In addition,no significant difference was detected in the level of probing depth or attachment loss before and after surgery.However,the level of plaque index at 3,6 and 12 months and the level of bleeding index 6 and 12 months after surgery were significantly lower than those before surgery.Conclusions:Laparoscopic sleeve gastrectomy can reduce the degree of periodontal inflammation in obese patients with T2DM to a certain extent.
