双层探测器光谱CT鉴别良恶性椎体压缩骨折的初步研究 被引量:2

摘要 目的:探讨双层探测器光谱CT鉴别诊断良恶性椎体压缩骨折的价值。方法:回顾性分析经病理证实或临床确诊的34例骨质疏松性椎体压缩骨折(OVCF)患者(OVCF组)和19例溶骨性转移瘤性椎体压缩骨折(MVCF)患者(MVCF组),2组均行双能量光谱CT扫描,计算ROI内120 kV混合能量、40~100 keV及200 keV单能量下的CT值、光谱曲线斜率(k值)及有效原子序数(Zeff值)。应用Mann Whitney U检验及ROC曲线比较2组数据差异性及鉴别诊断效能。结果:良性组中,40~60 keV单能量的CT值、k值及Zeff值均高于恶性组(均P<0.05),而常规120 kV混合能量及70~100 keV、200 keV单能量的CT值差异无统计学意义(P>0.05)。ROC曲线发现Zeff值的AUC最大,为0.887(临界值7.85),k值、40 keV单能量CT值的诊断效能较高。结论:双能量CT对鉴别椎体良恶性压缩骨折具有一定的应用价值。 Objective:To investigate the value of double-layer detector spectral CT in differential diagnosis of benign and malignant vertebral compression fractures.Methods:A retrospective analysis of 34 patients with osteoporotic vertebral compression fracture(OVCF group)and 19 patients with metastatic vertebral compression fracture(MVCF group)confirmed by pathology or clinical diagnosis was performed,and both groups were scanned by double-layer detector spectral CT.The ROIs’CT values at 120 kV mixed energy,40~100 keV and 200 keV single energy were calculated,and also the spectral curve slope(k value)and the effective atomic number(Zeff value)were calculated.The Mann Whitney U test and ROC curve were used to compare the data difference and the differential diagnostic efficiency between the two groups.Results:CT value,k value and Zeff value at 40~60 keV single energy of OVCF group were higher than those of MVCF group(all P<0.05),while CT value at 120 kV mixed energy,70~100 and 200 keV single energy had no significant differences between the two groips(all P>0.05).The ROC curve showed that AUC of Zeff value was the largest,0.887(critical value of 7.85),the k value and CT value at 40 keV single energy had higher diagnostic efficiency.Conclusion:Double-layer detector spectral CT has certain application value in distinguishing between benign and malignant vertebral compression fractures.
