摘要
目的 分析北京丰台医院门急诊处方,了解不合理用药情况及原因,以制定有效的改进措施,提高处方合格率,促进合理用药。方法 对北京丰台医院2019年12月份门急诊处方,采用智能审核系统审核和人工审核相结合的方式进行回顾性的处方点评,对存在的不合理情况进行整理评价,并依据帕累托图分析法总结产生不合理处方的主要因素和次要因素。结果 2019年12月份医院门急诊处方共计88 605张,不合理用药处方2 448张,占2. 76%。不合理用药的主要因素有用法用量不适宜、重复用药和给药途径不适宜,次要因素为适应证不适宜。结论 通过门急诊处方全样本点评,并进行帕累托图分析,有利于全面了解医院处方现状和具体问题,以便开展有针对性的合理用药培训工作,同时要注意及时更改和完善智能系统规则,逐步提高、优化系统前置审核处方的能力和效率,有效规范医师处方行为,保证患者安全用药。
Objective To analyze the outpatient and emergency prescriptions of Beijing Fengtai Hospital,understand the irrational use of drugs and the reasons,so as to formulate effective improvement measures,improve the qualified rate of prescriptions,and promote rational use of drugs.Methods To review the outpatient and emergency prescriptions of Beijing Fengtai Hospital in December 2019 by combining intelligent system audit and manual audit.Sort out and evaluate the existing unreasonable situations,and summarize the main and secondary factors of unreasonable prescriptions according to Pareto Diagram Analysis.Results In December 2019,there were 88605 hospital outpatient and emergency prescriptions,and 2448 prescriptions for irrational medication,accounting for 2.76%.The main factors for irrational use of drugs were inappropriate usage and dosage,repeated medication and inappropriate route of administration,and the secondary factor was inappropriate indications.Conclusion Through the evaluation of the full sample of outpatient and emergency prescriptions and Pareto diagram analysis,it is helpful to fully understand the current situation of hospital prescriptions and the specific problems in order to carry out targeted training on rational drug use.At the same time,pay attention to timely change and improve the rules of the intelligent system,gradually improve and optimize the system's ability and efficiency of pre-prescription audit,effectively standardizing physicians'prescription behavior,and ensuring patients'safe medication.
