摘要
目的:系统评价《伤寒杂病论》中理气活血类经方联合核苷(酸)类药物治疗慢性乙型肝炎(CHB)肝纤维化患者的临床疗效。方法:检索PubMed、Cochrane Library、Embase、中国生物医学数据库、中国知网、万方、维普数据库自建库至2020年5月的文献,筛选有关《伤寒杂病论》中理气活血类经方联合核苷(酸)类药物治疗慢性乙型肝炎肝纤维化患者的随机临床对照试验(RCT)。对纳入的文献进行资料提取和质量评价后,采用RevMan 5.3软件进行Meta分析。结果:纳入符合标准的文献共11篇,总计1006例患者,其中治疗组505例,对照组501例,试验组患者血清HA[SMD=-1.23,95%CI:-1.55~-0.91,P<0.001]、LN[SMD=-0.88,95%CI:-1.02~-0.74,P<0.001]、PC-Ⅲ[SMD=-1.14,95%CI:-1.28~-0.99,P<0.001]、Ⅳ-C[SMD=-0.71,95%CI:-0.85~-0.57,P<0.001]、ALT[MD=-24.51,95%CI:-35.18~-13.84,P<0.001]显著低于对照组,差异具有统计学意义。结论:理气活血类经方联合核苷(酸)类药物可有效抑制CHB患者肝纤维化进展,改善部分肝功能生化指标,且安全性较好。
Objective:To systematically review the clinical efficacy of classical prescription combined with nucleotide drugs for the treatment of liver fibrosis resulting from chronic hepatitis B.Methods:PubMed,the Cochrane Library,Embase,Chinese Biomedical Database,CNKI,WanFang,VIP were searched from inception to May 2020 to collect comprnhensively randomized controlled clinical trials(RCTs)on the treatment of patients with liver fibrosis resulting from chronic hepatitis B by combination of classical prescription in The Treatise on Febrile and Miscellaneous Diseases and nucleotide drugs.After data extraction and quality evaluation of the included literature,RevMan5.3 software was used for meta-analysis.Results:Total 11 RCTs were included,1006 patients involved.Among all 505 cases were in the treatment group,while the other 501 cases were in the control group.Meta-analysis results showed that the serum indexes including HA,LN,PC-Ⅲ,Ⅳ-C,ALT of the treatment group,were significantly lower than those of control group.HA[SMD=-1.23,95%CI:-1.55~-0.91,P<0.001],LNSMD=-0.88,95%CI:-1.02~-0.74,P<0.001),PC-ⅢSMD=-1.14,95%CI:-1.28~-0.99,P<0.001],Ⅳ-C[SMD=-0.71,95%CI:-0.85~-0.57,P<0.001],ALT[MD=-24.51,95%CI:-35.18~-13.84,P<0.001].Conclusion:Classical prescription combined with nucleotide drugs can effectively inhibit the progression of liver fibrosis in patients with chronic hepatitis B,improve biochemical indexes of liver function.
